摘要
目的 分析二维超声与剪切波弹性成像技术在先天性肌斜颈中的诊断价值。方法 随机选取80例2021年1月-2022年12月在甘肃省中医院针灸推拿科门诊就诊的疑似先天性肌性斜颈患儿,应用彩色超声诊断仪为全部患儿实施检查,选择二维超声及剪切波弹性成像模式进行检查,以病理诊断为“金标准”,对比二维超声以及二维超声联合剪切波弹性技术的诊断效能,比较患儿健侧与患侧短轴切面及长轴切面胸锁乳突肌厚度、弹性测值。结果 全部80例患儿中病理诊断确诊为先天性肌斜颈患儿47例,检出率为58.75%。二维超声联合剪切波弹性成像技术诊断准确度、特异度、敏感度以及阳性预测值、阴性预测值均高于二维超声,差异存在统计学意义(P<0.05)。先天性肌斜颈患儿短轴切面及长轴切面健侧应变比、剪切模量、剪切波速度、杨氏模量及厚度值均低于患侧,差异均有统计学意义(P<0.05)。结论 二维超声与剪切波弹性成像技术在先天性肌斜颈中的诊断效能较高,可有效降低临床漏诊风险以及误诊风险,能够为临床明确病变组织变化提供重要理论依据,有助于临床制定针对性病情干预和控制方案,值得临床应用。
Objective To analyze diagnostic value of two-dimensional ultrasound and shear wave elastography for congenital muscular torticollis.Methods The paper chose 80 children with suspected congenital muscular torticollis in acupuncture department of Gansu Provincial Hospital of Traditional Chinese Medicine from January 2021 to December 2022 randomly,examined with dimensional ultrasound and shear wave elastic imaging modes by color ultrasound diagnostic apparatus.With gold standard of pathological diagnosis,diagnostic effect of two-dimensional ultrasound and twodimensional ultrasound combined with shear wave elastic technology was compared,thickness and elasticity of sternocleidomastoid muscle on healthy side and affected side on short axis and long axis sections was compared.Results Of all 80 children,47 were diagnosed as congenital muscular torticollis by pathological diagnosis,with detection rate of 58.75%.Diagnostic accuracy,specificity,sensitivity,positive predictive value,and negative predictive value of two-dimensional ultrasound combined with shear wave elastography were significantly higher than two-dimensional ultrasound(P<0.05).Strain ratio,shear modulus,shear wave velocity,Young's modulus,and thickness of healthy side in both short and long axis sections was significantly lower than affected side(P<0.05).Conclusion Two-dimensional ultrasound and shear wave elastography are highly effective in diagnosis of congenital muscular torticollis,can reduce risk of clinical missed diagnosis and misdiagnosis effectively,provide important theoretical basis for clinical identification of pathological tissue changes,contribute to formulation of targeted disease intervention and control scheme,which is worthy of clinical application.
