摘要
目的研究呼吸机相关肺炎痰病原菌培养分布及易感因素logistics分析。方法选取我院2016年2月-2020年2月我院收治的重症监护室行机械通气治疗的181例患者为研究对象,将治疗期间出现呼吸机相关肺炎的患者分为阳性组(n=90例),未出现的分为阴性组(n=91)。分析阳性组痰病原菌培养分布;对比两组患者一般资料进行分析,以研究呼吸机相关肺炎研究易感因素。结果阳性组总共分离出112株病原菌,其中革兰阴性菌76株,占67.85%,以铜绿假单胞菌、肺炎克雷伯菌、鲍氏不动杆菌为主;革兰阳性菌23株,占20.53%,主要以金黄色葡萄球菌、肠球菌为主;真菌13株,占11.50%,主要以白色念珠菌和热带念珠菌为主。两组患者在年龄、抗生素种类、抗生素时间、机械通气时间、侵入性操作等方面均具有统计学意义(P<0.05);经Logisitic分析,年龄、抗生素种类、抗生素时间、机械通气时间、侵入性操作是呼吸机相关肺炎独立危险因素。结论呼吸机相关肺炎痰病原菌主要以革兰阴性菌为主,年龄、抗生素种类、抗生素时间、机械通气时间、侵入性操作均是呼吸机相关肺炎独立危险因素。
Objective To study the distribution of sputum pathogens in ventilator⁃associated pneumonia and logistics a⁃nalysis of susceptible factors.Methods One hundred and eighty⁃one patients treated with mechanical ventilation in the in⁃tensive care unit of our hospital from February 2016 to February 2020 were enrolled as the research objects.Patients who de⁃veloped ventilator⁃related pneumonia during treatment were usedas the positive groups(n=90 cases),and those without were as the negative group(n=91).The distribution of sputum pathogenic bacteria in the positive group was analyzed.The general data of the two groups of patients was compared to study the susceptibility factors of ventilator⁃associated pneumoni⁃a.Results A total of 112 pathogens were isolated in the positive group,of which 76(67.85%)were gram⁃negative bacteri⁃a,mainly including Pseudomonas aeruginosa,Klebsiella pneumoniae and Acinetobacter baumannii;23(20.53%)gram⁃posi⁃tive bacteria,mainly comprising of Staphylococcus aureus and Enterococcus;13(11.50%)fungi,mainly consisting of Candi⁃da albicans and Candida tropicalis.The two groups of patients were statistically significant in terms of age,antibiotic type,antibiotic time,mechanical ventilation time,invasive operation,etc.(P<0.05).Logisitic analysis showed that age,antibiot⁃ic type,antibiotic time,mechanical ventilation time,invasiveness operation were independent risk factors for ventilator⁃asso⁃ciated pneumonia.Conclusion The pathogens of ventilator⁃associated pneumonia sputum are mainly Gram⁃negative bacte⁃ria.Age,type of antibiotics,antibiotic time,mechanical ventilation time,and invasive operation are all independent risk fac⁃tors for ventilator⁃associated pneumonia.
