摘要
目的研究髓芯减压术治疗创伤性股骨头坏死与激素性股骨头坏死的疗效。方法回顾性选取2017年1月至2022年1月入河西学院附属张掖人民医院的80例股骨头坏死患者作为研究对象,根据患者不同疾病类型将其分为研究组(n=40)与对照组(n=40)。研究组为激素性股骨头坏死患者,有激素服用史,无其他股骨头坏死诱发因素;对照组为创伤性股骨头坏死患者,有明确创伤史;两组患者均接受髓芯减压术治疗。比较两组患者术前,术后3、6个月髋关节Harris评分与术后3个月的治疗总有效率、并发症(感染、深静脉血栓、活动受限)发生率。结果术后3、6个月,两组患者髋关节Harris评分均较术前明显升高,术后6个月髋关节Harris评分均较术后3个月明显升高,差异均有统计学意义(P<0.05);研究组患者术后3、6个月髋关节Harris评分与治疗总有效率分别为(85.81±2.47)分、(90.78±3.01)分、95.00%,均明显高于对照组[(81.53±2.36)分、(86.16±2.83)分、75.00%],差异均有统计学意义(P<0.05)。两组患者术后3个月感染、深静脉血栓、活动受限等并发症发生率比较,差异无统计学意义(P>0.05)。结论髓芯减压术治疗激素性股骨头坏死的临床疗效明显优于创伤性股骨头坏死,对激素性股骨头坏死患者髋关节功能的改善效果更为显著,且并发症少,安全性高。
Objective To investigate the efficacy of core decompression in the treatment of traumatic femoral head necrosis(ONFH)and hormonal ONFH.Methods By retrospective analysis,80 cases of ONFH patients admitted to Zhangye People's Hospital Affiliated to Hexi University from January 2017 to January 2022 were selected as research objects and divided into study group(n=40)and control group(n=40)according to different disease types.In the study group,40 patients had hormonal ONFH,had a history of hormone use,and had no other ONFH inducing factors.The control group had traumatic ONFH and a clear history of trauma.All patients in both groups received core decompression.The hip Harris score before surgery,3 months after surgery and 6 months after surgery,the total effective rate of 3 months after surgery and incidence of complications(infection,deep vein thrombosis and limited mobility)were compared.Results The hip Harris scores of the two groups at 3 and 6 months after surgery were significantly higher than those before surgery,the differences were statistically significant(P<0.05);the hip Harris scores 6 months after surgery were significantly higher than of 3 months after surgery,and the differences were statistically significant(P<0.05).The hip Harris scores at 3 months and 6 months after surgery and the total effective rate in the study group were(85.81±2.47)points,(90.78±3.01)points,95.00%,which were higher than those in the control group[(81.53±2.36)points,(86.16±2.83)points,75.00%],and the differences were statistically significant(P<0.05).There were no statistically significant differences in the incidence of infection,deep vein thrombosis and restriction of movement between the two groups 3 months after surgery(P>0.05).Conclusion The efficacy of core decompression in the treatment of patients with hormonal ONFH is significantly better than that of patients with traumatic ONFH,and the improvement of hip function in patients with hormonal ONFH is more significant,with fewer complications and high safety.
