DEMETICS超声智能辅助诊断系统联合ACRTI-RADS分类对甲状腺结节良恶性的诊断价值

摘要 目的研究DEMETICS超声智能辅助诊断系统结合美国放射学会(ACR)甲状腺影像报告与数据系统(TI-RADS)分类对甲状腺结节(TN)良恶性风险的评价作用。方法将我院2019年1月~2020年12月收治的400例TN患者纳入研究。分析不同良恶性TN的二维超声图像特征,以病理诊断结果为金标准,比较DEMETICS、ACR TI-RADS分类及二者联合诊断TN的效能,并比较不同性质TN患者血清内脂素以及脂联素水平。结果恶性结节边界模糊,形态不规则,纵横比≥1,内部低回声,后方回声减弱,微钙化和血流信号分级Ⅰ级占比均高于良性结节,而血流信号分级Ⅱ级、Ⅲ级占比均低于良性结节(P<0.05)。以病理结果为金标准,DEMETICS联合ACR TI-RADS分类诊断TN的灵敏度、特异度及准确度均高于DEMETICS或ACR TI-RADS分类单独诊断(P<0.05)。恶性结节患者血清内脂素水平高于良性结节患者,而脂联素水平低于良性结节患者(P<0.05)。结论DEMETICS联合ACR TI-RADS分类诊断TN恶性风险的价值较高。 Objective To study the role of DEMETICS ultrasound intelligent auxiliary diagnosis system combined with the American college of radiology(ACR)thyroid imaging reporting and data system(TI-RADS)classification in the evaluation of benign and malignant risk of thyroid nodule(TN).Methods A total of 400 patients with TN admitted to our hospital from January 2019 to December 2020 were included in this study.The two-dimensional ultrasound image features of different benign and malignant TN were analyzed,and the efficacy of DEMETICS,ACR TI-RADS classification and their combined diagnosis of TN was compared based on the pathological diagnosis results as the gold standard.In addition,the levels of serum adiponectin and adiponectin in patients with different TN properties were compared.Results Malignant nodules had fuzzy boundaries,irregular shapes,aspect ratio≥1,internal low echo,posterior echo was weakened,and the proportion of microcalcification and blood flow signal gradeⅠwas higher than that of benign nodules,while the proportion of blood flow signal gradeⅡ,Ⅲwas lower than that of benign nodules(P<0.05).Taking pathological results as the gold standard,the sensitivity,specificity and accuracy of DEMETICS combined with ACR TI-RADS classification in diagnosing TN were higher than those of DEMETICS or ACR TI-RADS classification alone(P<0.05).The serum adiponectin level in patients with malignant nodules was higher than that in patients with benign nodules,while the serum adiponectin level was lower than that in patients with benign nodules(P<0.05).Conclusion DEMETICS combined with ACR TI-RADS classification has high diagnostic value in TN.
