摘要
目的通过尿碘水平评估常州市武进区居民的总体碘营养状况,探讨碘营养状况与甲状腺功能之间的相关性。方法于2018年6月—2019年6月采集武进区14个乡镇,共900名成年受试者的清晨空腹尿碘浓度、甲状腺功能、甲状腺球蛋白(TG)、抗甲状腺球蛋白抗体(TGAb)及甲状腺过氧化物酶抗体(TPOAb)水平,剔除59例数据不全者。按甲状腺功能减退诊断标准将841民受试者分为甲减组12例,亚临床甲减组79例,甲功正常组750名。结果3组比较中,甲减组尿碘水平[265.2(165.56,284.56)μg/L]显著升高,差异有统计学意义(F=3.825,P=0.022)。甲减/亚临床甲减组TGAb、TPOAb双阳率(14.3%)较甲功正常组升高,差异有统计学意义(P<0.05)。不同甲状腺自身抗体组比较,TGAb及TPOAb均阳性组尿碘水平[124.99(103.11,178.08)μg/L]最低,差异有统计学意义(F=2.633,P=0.049)。尿碘水平与TPOAb呈负相关(r=-0.212,P=0.001)。结论常州市武进区居民总体碘营养状态适宜。碘摄入状况可能与自身免疫性甲状腺功能减退相关。
Objective In order to investigate the correlation between iodine nutrition and thyroid function,the overall iodine nutrition status of residents in Wujin district of Changzhou was evaluated by urinary iodine level.Methods From June 2018 to June 2019,fasting urine iodine concentration,thyroid function,thyroglobulin(TG),antithyroglobulin antibody(TGAb)and thyroid peroxidase antibody(TPOAb)levels were collected from nine hundred adult subjects in 14 townships of Wujin District.Fifty-nine cases with incomplete data were excluded.According to the diagnostic criteria of hypothyroidism,eight hundred and forty-one subjects were divided into hypothyroidism group(12 cases),subclinical hypothyroidism group(79 cases)and normal hypothyroidism group(750).Results Three groups in comparison urinary iodine level[265.2(165.56,284.56)μg/L]in hypothyroidism group was significantly increased,the difference was statistically significant(F=3.825,P=0.022).The double positive rate of TGAb and TPOAb(14.3%)in hypothyroidism/subclinical hypothyroidism group was higher than those in the normal hypothyroidism group,the difference was statistically significant(P<0.05).Comparison of different thyroid autoantibody,urinary iodine level[124.99(103.11,178.08)μg/L]in both TGAb and TPOAb positive groups was the lowest,the difference was statistically significant(F=2.633,P=0.049).Iodine level was negatively correlated with TPOAb(r=-0.212,P=0.001).Conclusion The overall iodine nutritional status of residents in Wujin District of Changzhou City is appropriate.The iodine intake may be related to autoimmune hypothyroidism.
