腹泻儿童非伤寒沙门菌的耐药性及毒力基因分析 被引量:4

摘要 目的探讨腹泻儿童非伤寒沙门菌血清型和耐药性及毒力基因的情况。方法收集苏州大学附属儿童医院2020年1月至2022年1月腹泻患儿粪培养标本中分离培养的71株非伤寒沙门菌作为研究对象,采用全自动质谱仪鉴定菌株,采用玻片凝集法进行血清学分型,纸片扩散法和全自动细菌鉴定药敏分析仪进行抗生素耐药分析。聚合酶链反应(PCR)检测,非伤寒沙门菌株的21种毒力基因和3种耐药基因。结果从腹泻患儿粪培养分离71株NTS血清型,以鼠伤寒沙门菌为主(52.11%),其次为肠炎沙门菌(14.08%)。NTS对氨苄西林的耐药率最高为88.73%。其次是哌拉西林(54.93%)和复方新诺明(40.85%),对头孢曲松和头孢哌酮的耐药率均为32.39%。亚胺培南耐药和中介各1株。鼠伤寒沙门菌对毒力基因sitC、sseL、sifA、siiE及phoP的携带率为100%。肠炎沙门菌对毒力基因sifA、siiE、phoP、pagN及pagC携带率高达90%。毒力基因spvB和prot6E常成对出现,主要分布在肠炎沙门菌。bapA和pefA的检测存在互补关系,bapA基因阳性则pefA阴性。结论苏州地区腹泻儿童非伤寒沙门菌血清型主要是鼠伤寒沙门菌和肠炎沙门菌,沙门菌对常用抗生素表现出较高水平的耐药性,且细菌携带多种毒力基因。 This study was aimed at investigating the serotypes,drug resistance and virulence genes of non-typhoid Salmonella in children of Soochow with diarrhea.A total of 71 strains of NTS were isolated from the feces of children with diarrhea from January 2020 to January 2022 at the Children’s Hospital of Soochow University.The strains were identified with a MALDI Biotyper,serological typing was performed with serum agglutination,and antibiotic resistance was analyzed with the K-B method and an automatic bacterial identification drug sensitivity analyzer.The 21 virulence genes and three drug resistance genes were detected by polymerase chain reaction(PCR).Among 71 NTS isolates,Salmonella typhimurium accounted for 52.11%and was followed by Salmonella enteritidis(14.08%).The drug resistance rate of NTS to ampicillin was highest(88.73%),and was followed by piperacillin(54.93%)and cotrimoxazole(40.85%).The resistance rate to ceftriaxone and cefoperazone was 32.39%.Notably,one NTS isolate was resistant to imipenem,and one isolate was intermediate.Virulence genes were present in all isolates of Salmonella typhimurium,including sitC,sseL,sifA,siiE and phoP.Salmonella enteritidis had a high carrier rate of 90%for virulence genes sifA,siiE,phoP,pagN and pagC.The virulence genes spvB and prot6E often appeared in pairs and were distributed mainly in Salmonella enteritidis.The detection of bapA and pefA indicated a complementary relationship:if the bapA gene was positive,the pefA gene was negative.Salmonella typhimurium and Salmonella enteritidis are the main serotypes of NTS in children with diarrhea from Suzhou.NTS shows high resistance to commonly used antibiotics,and the bacteria carry a variety of virulence genes.
