摘要
目的 探讨分析对重症肺炎患者采用美罗培南结合纤支镜肺泡灌洗治疗的临床疗效及对改善呼吸功能、血清炎性因子水平的作用。方法 随机选择曲阜市人民医院2021年1—12月收治的重症肺炎患者80例,以随机分配方式分为美罗培南治疗的对照组(n=40)与结合纤支镜肺泡灌洗治疗的观察组(n=40),对比两组临床疗效,并评估患者治疗前后呼吸功能及机体炎症反应。结果 观察组临床总体疗效较对照组高(90.00%vs 70.00%),差异有统计学意义(χ^(2)=5.000,P<0.05)。治疗后观察组PaO_(2)、PaO_(2)/FiO_(2)水平各为(73.52±7.35)mmHg、(323.37±30.62)mmHg,均较对照组高,而其PaCO_(2)水平为(44.52±4.45)mmHg,则较对照组低,差异有统计学意义(t=7.725、6.410、8.035,P<0.05)。治疗后观察组hs-CRP、PCT、WBC水平分别为(3.37±0.59)mg/L、(8.18±1.12)μg/L、(7.14±0.73)×10^(9)/L,均分别低于对照组,差异有统计学意义(t=13.107、13.029、20.765,P<0.05)。结论 对重症肺炎患者采用美罗培南结合纤支镜肺泡灌洗治疗可有效提高患者疗效,并能够改善患者的呼吸功能,控制其机体炎性反应,值得推广。
Objective To explore and analyze the clinical efficacy of meropenem combined with fiberoptic bronchoalveolar lavage in patients with severe pneumonia and the role of improving respiratory function and serum inflammatory factor levels.Methods A total of 80 patients with severe pneumonia were randomly selected from the Qufu People's Hospital from January to December 2021.They were randomly divided into meropenem treatment control group(n=40)and fiberoptic bronchoalveolar lavage treatment observation group(n=40).The clinical efficacy of the two groups was compared,and the respiratory function and the body´s inflammatory response was evaluated before and after treatment.Results The overall clinical efficacy rate of the observation group was higher than that of the control group(90.00%vs 70.00%),the difference was statistically significant(χ^(2)=5.000,P<0.05).After treatment,the levels of PaO2 and PaO2/FiO2 in the observation group were(73.52±7.35)mmHg and(323.37±30.62)mmHg,respectively,which were higher than those in the control group,the PaCO2 level was(44.52±4.45)mmHg,which was lower than that of the control group,the difference was statistically significant(t=7.725,6.410,8.035,P<0.05).After treatment,the levels of hs-CRP,PCT,and WBC in the observation group were(3.37±0.59)mg/L,(8.18±1.12)μg/L,(7.14±0.73)×10^(9)/L,respectively,which were lower than those in the control group,the difference was statistically significant(t=13.107,13.029,20.765,P<0.05).Conclusion Meropenem combined with bronchoscopic alveolar lavage for patients with severe pneumonia can effectively improve the curative effect of patients,improve the respiratory function of patients,and control the inflammatory response of the body,which is worthy of promotion.
