摘要
目的分析经跗骨窦切口应用微型钢板结合空心钉内固定在跟骨骨折治疗中的实施效果。方法择取2020年6月—2021年6月福建省南平市建阳第一医院收治的60例跟骨骨折患者作为研究对象,按照其术式不同分成对照组(n=30)与观察组(n=30)。对照组采用L形切口钢板内固定术治疗,观察组采用经跗骨窦切口微型钢板联合空心钉内固定术治疗。记录并对比两组手术相关指标、治疗效果、术后并发症发生状况。结果观察组手术时间、骨折愈合时间短于对照组,术中出血量少于对照组,差异有统计学意义(P<0.05)。观察组术后出现皮肤坏死、切口感染的概率较对照组要低,差异有统计学意义(P<0.05)。术后6个月随诊,观察组患肢Bohler角、Gissane角、跟骨高度、跟骨宽度与对照组比较,差异无统计学意义(P>0.05)。观察组AOFAS踝-后足功能评分优良率(93.33%),较对照组(73.33%)要高,差异有统计学意义(χ^(2)=4.320,P=0.038)。结论经跗骨窦切口微型钢板联合空心钉内固定治疗跟骨骨折具有手术时间短、并发症少、术后愈合快等优点,能够有效促进骨折复位,改善术后足踝功能。
Objective To analyze the effect of internal fixation of micro plate combined with hollow nail through tarsal sinus incision in the treatment of calcaneal fracture.Methods 60 patients with calcaneal fractures admitted to Jianyang First Hospital in Nanping City,Fujian Province from June 2020 to June 2021 were selected as the study subjects,and were divided into a control group(n=30)and an observation group(n=30)according to their surgical procedures.The control group was treated with L-shaped incision plate internal fixation,and the observation group was treated with tarsal sinus incision micro plate combined with hollow nail internal fixation.Operation related indexes,therapeutic effects and postoperative complications of the two groups were recorded and compared.Results operation time and fracture healing time in the observation group were shorter than those in the control group,intraoperative blood loss than the control group,the difference was statistically significant(P<0.05).The incidence of skin necrosis and incision infection in the observation group was lower than that in the control group,the difference was statistically significant(P<0.05).Six months after surgery,there was no statistically significant difference in Bohler angle,Gissane angle,calcaneal height and calcaneal width between the observation group and the control group(P>0.05).The good rate of AOFAS ankle-hind foot function score in the observation group was 93.33%,which was higher than that in the control group(73.33%),the difference was statistically significant(χ^(2)=4.320,P=0.038).Conclusion The treatment of calcaneal fracture by tarsal sinus incision with micro plate combined with hollow nail internal fixation has the advantages of short operation time,fewer complications and faster postoperative healing,which can effectively promote fracture reduction and improve postoperative foot and ankle function.
