摘要
目的探讨肺癌患者在免疫治疗期间的症状体验,为医护人员优化症状管理策略提供依据.方法采用现象学研究方法,选取16例免疫治疗期间的肺癌患者进行半结构式访谈,运用Colaizzi 7步分析法对资料进行分析.结果共归纳出3个主题,症状感知呈多样性:皮肤症状、消化系统症状、全身症状、呼吸系统症状、神经系统症状、骨关节和肌肉症状;造成多重影响:影响躯体运动功能、影响睡眠质量、影响情绪状态、影响日常生活、影响治疗计划;衍生出多种症状应对:寻求医护支持、自我应对、寻求家庭-社会支持、寻求信息支持.结论免疫治疗期间,肺癌患者面临着多种困扰,医护人员应加强症状评估和管理,关注患者的身心和社会功能变化,建立多元化支持体系,帮助患者有效应对症状及其产生的影响,提高生活质量.
Objective To investigate the symptom experience in lung cancer patients receiving immunotherapy,so as to provide a basis for medical staff to improve symptom management.Methods A qualitative phenomenological approach was used for this study.Sixteen lung cancer patients receiving immunotherapy were enrolled for semi-structured interviews and the data were analyzed by Colaizzi′s 7-step analysis method.Results Three core themes were extracted,multiple symptom experiences(skin symptoms,digestive symptoms,general symptoms,respiratory symptoms,neurological symptoms,osteoarticular and muscular symptoms),multiple effects(poor motor functions,poor sleep,negative emotions,daily lives were affected,treatment plans were replaned),and multiple coping styles(medical help-seeking,active self-coping,seeking help from family and friends,seeking information support).Conclusion During immunotherapy,lung cancer patients suffer from kinds of distress.Medical staff should strengthen symptom assessment and management,pay attention to the changes of patients′physical,psychological and social functions,and establish a diversified supporting system,so as to help patients effectively cope with those symptoms and related effects,and to improve their quality of life.
