摘要 目的探讨以问题为基础的教学方法(PBL)在呼吸与危重症医学科培训中的应用效果。方法选取解放军总医院第二医学中心呼吸与危重症医学科2021年6月至2022年3月接受同质化培训的受训医生48名,按照随机数字表法将其分为观察组和对照组,两组各24名,对照组采用传统教学法,观察组开展PBL教学方法进行老年吸入性肺炎(APIE)诊治培训,两组均接受培训4个月。比较培训结束后两组医生的理论知识考试、实践技能考核成绩、综合能力及教学满意度。结果所有受训医生全部通过考核,观察组医生的理论知识考试成绩、巴恩斯犹太医院脑卒中吞咽障碍筛查评估成绩、肺CT阅读能力成绩和纤维内镜吞咽检查操作成绩均高于对照组,差异有统计学意义(P<0.05)。观察组医生的自学能力、知识理解能力、临床思维能力、表达能力、教学氛围感受、知识掌握情况及对教学模式满意度评分均高于对照组,差异有统计学意义(P<0.05)。结论PBL教学方法可显著提升呼吸与危重症医学科医生的理论及实践技能考核成绩,提高APIE诊治的综合能力。 Objective To investigate the application effect of problem-based learning(PBL)teaching method in the training of Department of Respiratory and Critical Care Medicine.Methods A total of 48 doctors who received homogeneous training in the Department of Respiratory and Critical Care Medicine of the Second Medical Center of Chinese PLA General Hospital from June 2021 to March 2022 were selected and divided into observation group and control group according to the random number table method,with 24 doctors in each group.The control group was taught by the traditional teaching method,while the observation group was taught by PBL teaching method to master the diagnosis and treatment of aspiration pneumonia in the elderly(APIE).Both groups received training for 4 months.The scores of theoretical knowledge examination,practical skill test,comprehensive ability and teaching satisfaction of doctors from two groups were compared after the training.Results All the trained doctors passed the examination.The scores of theoretical knowledge examination,the Barnes Jewish Hospital stroke dysphagia screening assessment,reading ability of lung CT and fiberoptic endoscopy swallowing examination of the observation group were higher than those of the control group,and the differences were statistically significant(P<0.05).The scores of self-learning ability,ability of understand knowledge,ability of clinical thinking,expression ability,teaching atmosphere,knowledge mastery and satisfaction with teaching model of the observation group were higher than those of the control group,and the differences were statistically significant(P<0.05).Conclusion PBL teaching method can significantly improve the theoretical and practice performance,and enhance the comprehensive ability of the diagnosis and treatment of APIE.
