摘要
目的探究高压氧应用于妊娠期高血压疾病患者的效果。方法选取109例妊娠期高血压疾病患者,将其随机分为对照组(54例)和观察组(55例)。在常规治疗的基础上,对照组行氧舱外高流量给氧,观察组在对照组治疗的基础上增加高压氧治疗,两组患者均治疗10 d。比较两组患者治疗前后的血压、脐动脉血流指标和血管内皮功能指标。结果治疗前,两组患者收缩压、舒张压、平均动脉压水平差异均无统计学意义(均P>0.05);治疗10 d后,观察组的收缩压、舒张压、平均动脉压水平均低于对照组(均P<0.05)。治疗前,两组患者脐动脉收缩期峰值/舒张末期最低速度、阻力指数、搏动指数差异均无统计学意义(均P>0.05);治疗10 d后,观察组的脐动脉收缩期峰值/舒张末期最低速度、阻力指数、搏动指数均小于对照组(均P<0.05)。治疗前,两组患者血清内皮素-1、一氧化氮水平差异均无统计学意义(均P>0.05);治疗10 d后,观察组血清内皮素-1水平低于对照组,血清一氧化氮水平高于对照组(均P<0.05)。结论高压氧治疗可有效地改善妊娠期高血压疾病患者的脐动脉血流情况和血管内皮功能,降低血压水平。
Objective To investigate the effect of hyperbaric oxygen in patients with hypertensive disorders in pregnancy.Methods A total of 109 patients with hypertensive disorders in pregnancy were selected and randomly divided into a control group(54 cases)or an observation group(55 cases).On the basis of routine treatment,the control group received high-flow oxygen therapy outside the oxygen chamber,based on which the observation group received hyperbaric oxygen treatment additionally.Both groups were treated for 10 days.The blood pressure,umbilical artery blood flow indexes,and vascular endothelial function indexes were compared between the two groups before and after treatment.Results Before treatment,there was no statistically significant difference in the systolic blood pressure,diastolic blood pressure,or mean arterial pressure level between the two groups(all P>0.05).After 10 days of treatment,the systolic blood pressure,diastolic blood pressure,and mean arterial pressure levels in the observation group were lower than those in the control group(all P<0.05).Before treatment,there was no statistically significant difference in the peak-systolic velocity/end-diastolic minimum velocity,resistance index,or pulsatility index of umbilical artery between the two groups(all P>0.05).After 10 days of treatment,the peak-systolic velocity/end-diastolic minimum velocity,resistance index,and pulsatility index of umbilical artery in the observation group were lower than those in the control group(all P<0.05).Before treatment,there was no statistically significant difference in the level of serum endothelin-1 or nitric oxide between the two groups(all P>0.05).After 10 days of treatment,the serum endothelin-1 level in the observation group was lower than that in the control group,while the serum nitric oxide level was higher than that in the control group(all P<0.05).Conclusion Hyperbaric oxygen treatment can effectively improve the umbilical artery blood flow and vascular endothelial function and reduce blood pressure levels in patients with hypertensive disorders in pregnancy.
