显微修复结合微型锚钉与单纯微型锚钉治疗锤状指效果比较

摘要 目的通过显微精细修复结合微型锚钉与单纯微型锚钉治疗锤状指的临床疗效差异比较,探索治疗锤状指的最优治疗方法。方法根据手术方式不同将2020年1月至2021年9月收治的44例锤状指患者分为甲、乙2组,甲组采用单纯微型锚钉的治疗方法,乙组采用显微精细修复结合微型锚钉的治疗方法,分别记录患者的围手术期的各项情况及术后手指活动度、并发症等情况,并按Crawford功能评定标准评估其临床疗效。结果术后所有患者都得到随访,随访时间6~15个月,平均7.3个月,术后3个月功能的优良率乙组优于甲组,差异有统计学意义(P<0.05),术后6个月功能的优良率2组之间差异无统计学意义(P>0.05);甲组在总并发症的发生率上高于乙组,差异有统计学意义(P<0.05),乙组在手术切口Ⅰ期愈合率上明显优于甲组,差异有统计学意义(P<0.05)。结论使用显微精细修复结合微型锚钉的方法治疗锤状指操作精准细致,固定牢固可靠,恢复良好,在减少术后并发症方面明显优与单纯微型锚钉的方法,值得在临床加以推广应用。 Objective To investigate the clinical efficacy of microsurgical repairment combined with micro arc bone anchor on repairing special type mallet finger.Methods From January 2020 to September 2021,totally 23 patients with special type mallet finger who were treated with microsurgical repairment combined with micro arc bone anchor were recruited.The postoperative therapeutic outcomes were assessed by Dargan evaluation criteria and Crawford evaluation criteria.Results All of 23 patients were postoperatively followed up for 7.6(3~15)months.The incision was well healed,achieving the healing by first intention.No complications such as joint stiffness,skin necrosis or finger deformity occurred.According to the Dargan evaluation criteria,there were 14 cases of excellent,8 of good and 1 of fair,with the excellent and good rate of 95.6%.According to the Crawford evaluation criteria,there were 15 cases of excellent,7 of good and 1 of fair,with the excellent and good rate of 95.6%.Conclusion The operation of microsurgical repairment combined with micro arc bone anchor provides a clear tissue identification,less invasive,firm fixation,fewer complication,more efficient surgical method for repairing special type mallet finger and embodies the concept of precision therapy,which is worthy of clinical promotion.
