胸腹部肿瘤^(125)I粒子植入术后不同位点的辐射剂量检测与意义

摘要 目的探讨胸腹部肿瘤患者不同数量^(125)I粒子植入术后24 h的辐射剂量及安全距离,为医护与陪护人员的防护提供参考。方法选取行^(125)I粒子植入的胸腹部肿瘤患者100例。根据植入^(125)I粒子的数量,采用三分法将患者分为少量组、中量组、大量组。采用γ射线辐射仪分别测量手术侧和对侧0 m、1.0 m、1.5 m、2.0 m距离的辐射剂量并进行重复测量方差分析。结果在手术侧与对侧测量中,粒子植入数量分组的组间效应显著(P<0.001),测量距离的主效应显著(P<0.001),且分组与测量距离存在显著交互效应(P<0.001)。手术侧不同距离的辐射剂量也显著大于对侧(P<0.001),且与分组存在显著交互效应(P<0.001)。2.0 m以上的辐射剂量达到国家标准公众限值(2.50μSv/h),1.0 m以上的辐射剂量达到国家标准职业限值(25.00μSv/h)。结论^(125)I粒子植入术后,距离越远防护效果越好,一般人员在离患者2.0 m以上、医护人员在离患者1.0 m以上可无需额外防护,且在患者对侧活动可进一步减少辐射剂量。 Objective To investigate the radiation dose and safe distance of 24 hours after different amounts of ^(125)I seeds implantation in patients with thoracic-abdominal tumors,so as to provide reference for the protection of medical and accompanying personel.Methods A total of 100 patients with thoracic-abdominal tumors treated with ^(125)I seeds implantation were selected.According to the number of ^(125)I seeds implanted,the patients were divided into small group,medium group and large group by trichotomy.The radiation dose at the distance of 0 meter,1.0 meter,1.5 meters and 2.0 meters away from the operative side and contralateral side of patients were detected byγ-ray radiometer,and the repeated measurement analysis of variance was performed.Results In the detection of the operative side and the contralateral side,there was significant intergroup effect on the number of implanted seeds(P<0.001),a significant main effect on the detection distance(P<0.001),and a significant interaction effect between the grouping and the detection distance(P<0.001).The radiation dose at the different distances away from the operative side were also significantly higher than those away from the contralateral side(P<0.001),which has a significant interaction effect with the grouping(P<0.001).The radiation dose at the distance above 2.0 meters reached the national standard public limit(2.50μSv/h),and the radiation dose at the distance above 1 meter reached the national standard occupational limit(25.00μSv/h).Conclusion After ^(125)I seeds implantation,the farther the distance,the better the protective effect.The general staff above 2.0 meters away from the patients,medical staff above 1.0 meter away from patients need no additional protection,and the activity on the contralateral side of the patient can further reduce the radiation dose.
