摘要
目的分析慢性支气管炎肺气肿(简称:慢支肺气肿)合并肺炎的X线影像学特点,为慢支肺气肿合并肺炎的临床诊疗提供帮助。方法选取2020年1月-2021年1月滕州市中心人民医院100例慢支肺气肿合并肺炎患者作为观察对象,均行X线检查,分析其X线影像学特点。结果慢支肺气肿合并肺炎X线影像特点以斑片状阴影、密度增高、囊状减低区、片状阴影区等为主,特征明显。本次研究中,100例患者共有90例X线检查图像阴影区密度增高,且阴影多呈斑片状、网状,同时内部有密度减低区,其余10例患者X线图像呈片状阴影区,沿叶或沿段分布,密度增高。慢支肺气肿合并肺炎致病菌主要为革兰阴性杆菌(G^(-)菌)、革兰阳性杆菌(G^(+)菌),其中以G^(-)菌最为常见,G^(-)菌检出率为70.00%,高于G^(+)菌的50.00%,差异有统计学意义(χ^(2)=8.333,P<0.05)。结论X线是检查和诊断慢支肺气肿合并肺炎重要手段,其影像学特点以斑片状阴影、密度增高、囊状减低区等为主,致病菌检测多为G^(-)菌,例如大肠杆菌、不动杆菌等,通过X线影像学分析可为制订治疗方案提供指导,具有临床推广价值。
Objective To analyze the X-ray imaging characteristics of chronic bronchitis emphysema ocmplicated with pneumonia,in order to provide help for the clinical diagnosis and treatment of chronic bronchitis emphysema complicated with pneumonia.Methods From January 2020 to January 2021,100 patients with chronic bronchitis emphysema complicated with pneumonia treated in Tengzhou Central People's Hospital were selected as the objects of observation.All patients underwent X-ray examination,and their X-ray imaging characteristics were analyzed.Results The X-ray imaging of chronic bronchitis emphysema complicated with pneumonia were characterized by patchy shadows,increased density,cystic reduced areas,patchy shadow areas,etc.The characteristics were obvious.In this study,90 cases of 100 patients had increased density in the shadow areas of X-ray images,and the shadows were mostly patchy and reticular,while there were areas with reduced density inside.The other 10 patients had patchy shadow areas distributed along the lobe or the segment,the density increased.The pathogenic bacteria of chronic bronchitis emphysema with pneumonia were mainly gram-negative bacilli(G^(-)bacteria)and gram-positive bacilli(G^(+)bacteria),among which Gbacteria were the most common,the detection rate of G^(-)bacteria was 70.00%,which was higher than 50.00%of G^(+)bacteria,and the difference was statistically significant(χ^(2)=8.333,P<0.05).Conclusion X-ray was an important means to check and diagnose chronic bronchitis emphysema complicated with pneumonia.Its imaging features mainly included patchy shadows,increased density and cystic reduced areas.The detection of pathogenic bacteria was mostly G^(-)bacteria,such as Escherichia coli,acinetobacter,etc.X-ray imaging analysis could provide guidance for formulating treatment plans,and had clinical promotion value.
