探讨改良Proseal喉罩联合支气管封堵器控制通气方式在无痛电子支气管镜检查中的应用

Exploring the Application of Modified Proseal Laryngeal Mask Combined with Bronchial Blocker to Control Ventilation Method in Painless Electronic Bronchoscopy

摘要 目的探究与分析改良Proseal喉罩联合支气管封堵器控制通气方式在无痛电子支气管镜检查中的应用。方法选取2020年2月—2022年2月黑龙江省医院收治的无痛电子支气管镜检查患者110例为研究对象,采取随机数表法分为两组,各55例。对照组给予普通改良喉罩控制通气,观察组给予改良Proseal喉罩联合支气管封堵器控制通气,对比两组患者麻醉前后血流动力学指标变化、气道管理效果及不良反应。结果与T0相比,两组T1时间点的收缩压(SBP)、舒张压(DBP)、心率(HR)较高,在T2时间点降低,差异有统计学意义(P<0.05)。对照组气道封闭压为(24.20±5.99)cmH_(2)O、喉罩通气优良率为76.36%、无痛电子支气管镜评估有效率为72.73%、不良反应发生率为14.55%,观察组气道封闭压为(35.36±5.74)cmH_(2)O、喉罩通气优良率为100.00%、无痛电子支气管镜评估有效率为100.00%、不良反应发生率为1.82%,观察组的气道封闭压、喉罩通气优良率、无痛电子支气管镜评估有效率均高于对照组,并发症发生率低于对照组,差异有统计学意义(t=9.976,χ^(2)=14.742、17.368、4.356,P<0.05)。结论改良Proseal喉罩联合支气管封堵器控制通气方式在无痛电子支气管镜检查中的应用可保证麻醉期血流动力学指标的稳定性,提高气道管理效果,降低不良反应发生风险,安全性较高。 Objective To investigate and analyze the application of modified Proseal laryngeal mask combined with bronchial blocker to control ventilation method in painless electronic bronchoscopy.Methods A total of 110 patients with painless electronic bronchoscopy admitted to systolic blood pressure(SBP)Heilongjiang Provinced Hospital from February 2020 to February 2022 were selected as the research objects and divided into two groups with 55 patients in each group by random number table method.The control group was given modified laryngeal mask control ventilation,and the observation group was given modified Proseal laryngeal mask combined with bronchial sealer control ventilation.The hemodynamic index changes,airway management effects and adverse reactions were compared between the two groups before and after anesthesia.Results compared with the time of to,in both groups,systolic blood pressure(SBP)was higher,diastolic blood pressure(DBP)was higher and heart rate(HR)was higher at T1 time point and decreased at T2 time point,and the difference was statistically significant(P<0.05).In the control group,the airway seal pressure was(24.20±5.99)cmH_(2)O,the good and good rate of laryngeal mask ventilation was 76.36%,the effective rate of painless electronic bronchoscopy was 72.73%,and the incidence of adverse reactions was 14.55%.In the observation group,airway seal pressure was(35.36±5.74)cmH_(2)O,the good and good rate of laryngeal mask ventilation was 100.00%,the effective rate of painless electronic bronchoscopy was 100.00%,and the incidence of adverse reactions was 1.82%.The airway seal pressure,the good and good rate of laryngeal mask ventilation and the effective rate of painless electronic bronchoscopy in the observation group were all higher than those in the control group,the incidence of complications was lower than that of control group,and the difference was statistically significant(t=9.976,χ^(2)=14.742,17.368,4.356,P<0.05).Conclusion The application of modified Proseal laryngeal mask combined with bronchosealer to control ventilation in painless electronic bronchoscopy can ensure the stability of hemodynamic indexes during anesthesia,improve the effect of airway management,reduce the risk of adverse reactions,and have high safety.
