摘要
目的 探讨不同浓度罗哌卡因在超声引导下肌间沟臂丛神经阻滞中的应用价值。方法 简单随机选取2020年1月—2021年12月在兴化市人民医院开展超声引导下肌间沟臂丛神经阻滞的患者100例为研究对象,以随机数表法分为两组,对照组(n=50)予以浓度为0.3%的罗哌卡因注射,观察组(n=50)予以浓度为0.5%的罗哌卡因注射。观察比较两组患者的麻醉指标、麻醉效果、安全性。结果 与对照组相比,观察组麻醉、运动阻滞起效时间更短,运动阻滞和感觉恢复时间、镇痛持续时间更长,差异有统计学意义(P<0.05)。观察组麻醉优良率为92.0%,高于对照组的76.0%,差异有统计学意义(χ^(2)=4.762,P<0.05)。两组不良反应发生率比较,差异无统计学意义(P>0.05)。麻醉前,两组各项体征指标(心率、平均动脉压)比较,差异无统计学意义(P>0.05);麻醉后,两组体征指标均下降,但两组间体征指标比较,差异无统计学意义(P>0.05)。结论选择浓度为0.5%的罗哌卡因对患者实施超声引导下肌间沟臂丛神经阻滞,能够提升麻醉安全性以及效果,不会增加对患者体征指标的影响程度。
Objective To investigate the application value of different concentrations of ropivacaine in ultrasoundguided interscalene brachial plexus block.Methods 100 patients who underwent ultrasoundguided intramuscular plexus block at Xinghua People's Hospital from January 2020 to December 2021 were randomly selected as the study subjects.They were randomly divided into two groups using a random number table method.The control group(n=50)was injected with 0.3%ropivacaine,while the observation group(n=50)was injected with 0.5%ropivacaine.Observed and compared the anesthesia indicators,anesthesia effects,and safety of two groups of patients.Results Compared with the control group,the observation group had shorter onset time for anesthesia and motor block,longer recovery time for motor block and sensation,and longer duration of pain relief,and the difference was statistically significant(P<0.05).The excellent and good anesthesia rate in the observation group was 92.0%,higher than 76.0%in the control group,and the difference was statistically significant(χ^(2)=4.762,P<0.05).There was no statistically significant difference in the incidence of adverse reactions between the two groups(P>0.05).There was no statistically significant difference in various physical indicators(heart rate,mean arterial pressure)between the two groups before anesthesia(P>0.05);after anesthesia,both groups showed a decrease in physical indicators,but there was no statistically significant difference in the comparison of physical indicators between the two groups(P>0.05).Conclusion Choosing 0.5%ropivacaine to perform ultrasoundguided interscalene brachial plexus block on patients can improve the safety and effect of anesthesia without increasing the impact on the patient´s physical signs.
