摘要
目的研究蜡样芽孢杆菌感染患者的临床特征。方法收集2018年1月至2021年12月苏州大学附属第一医院的蜡样芽孢杆菌患者的临床资料;按照感染部位的不同,将其分为血流感染组(BSI组)和非血流感染组(NBSI组),对两组患者的基础疾病、白细胞计数、中性粒细胞计数、C-反应蛋白、白蛋白及治疗和预后等进行分析。应用SPSS25.0统计软件进行数据分析。结果研究共纳入31名患者,14例菌血症、17例非菌血症。BSI组与NBSI组患者在性别(P=0.679)、平均住院时长(P=0.225)、所属病区(P=0.238)、侵入性操作(P=0.533)、医院内感染(P=0.928)、2周内抗菌药物暴露(P=0.211)、白细胞增高(P=0.486)差异均没有统计学意义;在年龄(P=0.009)、合并基础疾病(P=0.015)、存在粒细胞减少(P=0.024)、存在粒细胞缺乏(P=0.019)差异有统计学意义。BSI组患者平均年龄低于NBSI组,BSI组中性粒细胞减少、缺乏比例明显高于NBSI组;血液恶性肿瘤,实体肿瘤接受细胞毒性化疗和实体器官移植等免疫缺陷是该菌感染的重要基础。两组均经验性使用抗生素,但BSI组死亡率高于NBSI组。结论蜡样芽胞杆菌系统性感染非罕见且涉及多感染部位,菌血症患者较其他感染部位患者预后差。临床需要重视蜡样芽孢杆菌感染。
Objective To study the clinical characteristics of bacillus cereus infection.Methods Clinical data of Bacillus cereus patients in the First Affliated Hospital of Soochow University from January 2018 to December 2021 were collected.According to the different infection sites,patients were divided into bloodstream infection group(BSI group)and non-bloodstream infection group(NBSI group).The basic diseases,white blood cell count,neutrophil count,C-reactive protein,albumin,treatment and prognosis of patients in the two groups were analyzed.SPSS 25.0 statistical software was used for data analysis.Results A total of 31 patients were enrolled,including 14 bacteremia and 17 non-bacteremia.BSI group and NBSI group showed differences in gender(P=0.679),average length of hospital stay(P=0.225),area of disease(P=0.238),invasive operation(P=0.533),nosocomial infection(P=0.928),antibiotic exposure within 2 weeks(P=0.211)and leukocytosis(P-0.486)There was no statistical significance;There were statistically significant differences in age(P-0.015),combined with underlying diseases(P-0.015),presence of granulocytopenia(P=0.024)and presence of granulocytopenia(P-0.019).The mean age of patients in BSI group was lower than that in NBSI group,and the proportion of neutropenia and neutropenia in BSI group was significantly higher than that in NBSI group.Hematological malignancies,solid tumors receiving cytotoxic chemotherapy and solid organ transplantation are the important basis of the infection of this bacterium.Both groups were treated with antibiotics empirically,but the mortality of BSI group was higher than that of NBSI group.Conclusion Systemic infection of Bacillus cereus is not rare and involves multiple infection sites.Patients with bacteremia have a worse prognosis than those with other infection sites.Clinical attention should be paid to the infection of Bacillus cereus.
