基于回授法的健康教育模式对食管癌患者术后配合度及心理健康的影响 被引量:1

摘要 目的探讨基于回授法的健康教育模式对食管癌患者术后恢复的影响。方法将121例食管癌患者按照入院顺序分为研究组61例与对照组60例。对照组患者给予常规护理,研究组患者在对照组基础上给予基于回授法的健康教育模式。比较两组患者干预后术后配合度,干预前后分别采用心理状态症状自评量表评估患者心理健康状况、健康促进策略量表评估患者自我管理效能感、自制食管癌术后护理知信行水平调查表评估术后护理知信行水平、癌症患者生活质量测定量表评估患者生活质量,并比较两组患者术后并发症发生率。结果研究组患者术后配合度高于对照组,差异有统计学意义(P<0.05)。干预后研究组患者心理状态症状自评量表评分低于对照组,差异有统计学意义(P<0.01)。干预后研究组患者健康促进策略量表各维度评分高于对照组,差异有统计学意义(P<0.01)。干预后研究组患者食管癌术后护理知信行水平调查表各维度评分高于对照组,差异有统计学意义(P<0.01)。研究组癌症患者生活质量测定量表评分低于对照组,差异有统计学意义(P<0.01)。研究组患者术后并发症发生率低于对照组,差异有统计学意义(P<0.05)。结论食管癌患者术后给予基于回授法的健康教育模式后,患者不仅配合度提高,自我管理效能感增强,并发症减少,而且心理健康水平提高,知信行水平增强,生活质量改善,具有良好护理效果。 Objective To explore the influence of health education model based on teach-back method(TBM)on postoperative recovery of patients with esophageal cancer(EC).Methods A total of 121 EC patients were as assigned to research(n=61)and control group(n=60)according to admission order.Control group received routine nursing,the research was plus health education model based on the TBM on the basis of control group.Postoperative cooperation degrees were compared between two groups,before and after intervention psychological health states assessed with the Self-report Symptom Inventory-90,SCL-90,self-management efficacy with the Strategies Used by People to Promote Health(SUPPH),knowledge-belief-practice(KBP)levels of postoperative care with the self-made inventory for knowledge-belief-practice of postoperative care(SIKBP),qualities of life with the Quality of Life Questionnaire-core 30(QOL-C30),and incidences of post-operative complications compared between two groups.Results The postoperative cooperation was higher in research than in control group and difference statistically significant(P<0.05).After intervention scores on the SCL-90 were lower in research than in control group and difference statistically significant(P<0.01).After intervention scores on the SUPPH were higher in research than in control group and difference statistically significant(P<0.01).After intervention scores on all dimensions of the SIKBP were higher in research than in control group and difference statistically significant(P<0.01).After intervention scores on the QOL-C30 were lower in research than in control group and difference statistically significant(P<0.01).The incidences of postoperative complications were lower in research than in control group and difference statistically significant(P<0.05).Conclusion TBM-based health education model could raise patients’cooperation,enhance self-management efficacy,lower occurrence of complications,improve patients’mental health levels,enhance KBP levels,improve quality of life,and has good nursing efficacies.
