短时程脊髓电刺激治疗战士慢性睾丸疼痛的临床疗效观察

摘要 目的探讨短时程脊髓电刺激(TSCS)对战士慢性睾丸疼痛(CTP)的临床疗效。方法回顾性分析中国人民解放军联勤保障部队第九四〇医院疼痛科自2020年1月至2021年9月收治的战士CTP资料,经保守治疗无效后行TSCS植入治疗,植入时间为1~2周。收集患者术前及术后4、8、12、16、20、24周各时间段视觉模拟疼痛量表(VAS)、匹兹堡睡眠质量指数(PSQI)、术后各时间段药物及相应治疗情况,收集患者术后白细胞计数(WBC)、中性粒细胞(NEU)、淋巴细胞计数(LYM),感染检测指标降钙素原(PCT)、白细胞介素6(IL-6)水平及相关不良反应情况,进一步综合评估TSCS治疗效果。结果所有战士经TSCS治疗后VAS和PSQI均有明显下降(P<0.05);术后药物使用量均下降,术后4周内疼痛症状改善最明显,在术后12、16、20、24周后仍遗留疼痛患者逐渐减少。治疗过程中无植入部位感染及TSCS脱出等不良反应发生,经综合评估TSCS治疗战士CTP的有效率为85.00%。结论TSCS是一种治疗战士CTP非常安全有效的微创技术,值得临床推广使用。 Objective To investigate the clinical effect of short-term spinal cord electrical stimulation(TSCS)on chronic testicular pain(CTP)in soldiers.Methods The data of patients with CTP admitted to the pain department of the 940th Hospital of the Joint Logistics support Force of the Chinese people’s Liberation Army from January 2020 to September 2021 were retrospectively selected.After conservative treatment failed,the patients were treated with TSCS implantation for 1~2 weeks.The visual analogue pain score(VAS),pittsburgh sleep quality index(PSQI),drugs and corresponding treatment were collected before operation and at 4,8,12,16,20 and 24 after operation.White blood cell(WBC),neutrophil(NEU),lymphocyte count(LYM),procalcitonin(PCT),interleukin-6(IL-6)and related adverse reactions were collected to further evaluate the therapeutic effect of TSCS.Results VAS and PSQI of all soldiers decreased significantly after TSCS treatment(P<0.05),and postoperative drug use decreased.The pain symptoms were most significantly improved within 4 weeks after operation,and the number of patients with residual pain decreased gradually after 12 weeks,16 weeks,20 weeks and 24 weeks after operation.No adverse reactions such as implant site infection and TSCS prolapse occurred during the treatment.After comprehensive evaluation,the effective rate of TSCS in the treatment of young male CTP reached 85%.Conclusion TSCS is a safe and effective minimally invasive technique for the treatment of young male CTP and is worthy of clinical application.
