摘要
目的探讨清热化痰活血方加减治疗急性缺血性卒中的疗效及其抗炎作用。方法选取2014年1月-2016年12月无锡市中医医院及宿迁市中医院60例急性缺血性卒中患者,采用随机数字表法将患者分为研究组(30例,接受常规西药联合清热化痰活血方加减治疗)与对照组(30例,接受常规西药治疗),疗程10 d。对比2组治疗前后的中医证候积分、脑卒中专用生活质量量表(SS-QOL)、白细胞计数(WBC)、Fugl-Meyer运动功能评定量表(FMA)、中性粒细胞比率(NEUT)、美国国立卫生研究院卒中量表(NIHSS)、C-反应蛋白(hs-CRP)水平,临床疗效及不良反应。结果治疗前2组患者的中医证候积分、SS-QOL、NEUT、FMA、WBC、NIHSS、hs-CRP比较,差异无统计学意义(P>0.05);治疗后研究组的hs-CRP、WBC、NEUT水平低于对照组,NIHSS评分低于对照组,SS-QOL、FMA评分高于对照组,差异具有统计学意义(P<0.05);研究组总有效率高于对照组(P<0.05);2组的总不良反应率差异无统计学意义(P>0.05)。结论清热化痰活血方加减治疗可有效改善急性缺血性卒中患者神经功能,减轻患者局部炎症反应并提高患者生活质量。
Objective To investigate the effi cacy and anti-infl ammatory eff ect of the modifi ed Qingre Huatan Huoxue Decoction in the treatment of acute ischemic stroke(AIS).Methods A total of 60 patients with AIS in Suqian Hospital of Traditional Chinese Medicine from January 2014 to December 2016 were randomly divided into a study group(30 cases,treated with conventional western medicine combined with the modifi ed Qingre Huatan Huoxue Decoction for 10 days)and a control group(30 cases,treated with conventional western medicine for 10 days).The TCM symptom scores,scores of the Stroke Specifi c Quality of Life Scale(SS-QOL),whiTe blood cell counts(WBC),scores of the Fugl Meyer Motor Function Assessment Scale(FMA),neutrophil ratios(NEUT),scores of the National Institutes of Health Stroke Scale(NIHSS),levels of C-reactive protein(hs-CRP),clinical effi cacy and adverse reactions were compared between the two groups before and after treatment.Results There was no signifi cant diff erence in the TCM symptom scores,SS-QOL,WBC,FMA,NEUT,NIHSS and hs-CRP between the two groups before treatment(P>0.05).After treatment,the levels of WBC,the scores of hs-CRP,NEUT and NIHSS in the study group were significantly lower than those in the control group,while the scores of SS-QOL and FMA were signifi cantly higher than those in the control group(P<0.05).The total eff ective rate of the study group was higher than that of the control group(96.67%vs 80.00%)(P<0.05).There was no significant difference in the total adverse reaction rate between the two groups(13.33%vs 6.67%)(P>0.05).Conclusion The modifi ed Qingre Huatan Huoxue Decoction can eff ectively improve the neurological function of AIS patients,alleviate their local infl ammatory reactions,and improve their quality of life.
