高频超声与肌电图在腕管综合征中的应用价值 被引量:6

摘要 目的分析高频超声与肌电图在腕管综合征中的应用价值。方法回顾性将2020年4月至2022年4月河北医科大学第三医院就诊的腕管综合征患者98例纳入观察组,另择同期健康体检者129例作为对照组。分析2组高频超声测量指标、肌电图正中神经运动支、感觉支指标,分析高频超声检测正中神经腕管综合征的影像学图片。结果观察组左右径、前后径、横截面积均高于对照组(P<0.05)。观察组正中神经腕点、肘点的复合肌肉动作电位(CMAP),正中神经感觉支中指、食指、拇指的感觉传导速度(SCV)、感觉神经动作电位(SNAP)均低于对照组(P<0.05)。高频超声影像学图片显示健康人群正常正中神经横截面积相对较小,腕管综合征患者正中神经腕部增粗,横截面积明显增大。结论高频超声、肌电图对腕管综合征可进行形态和功能的定量分析,可作为临床诊治依据。 Objective To analyze the application value of high-frequency ultrasonography and electromyography in the carpal tunnel syndrome(CTS).Methods A total of 98 patients with CTS admitted to the Third Hospital of Hebei Medical University from April 2020 to April 2022 were retrospectively included in the observation group,and 129 healthy people were selected as the control group during the same period.Data of high-frequency ultrasonography,median nerve motor branch and sensory branch of electromyography were compared,and the imaging findings of the median nerve in CTS patients detected by high-frequency ultrasonography were analyzed.Results The left and right diameters,anterior and posterior diameters and cross sectional area in the observation group were significantly higher than those in the control group(P<0.05).The compound muscle action potential(CMAP)of the wrist and elbow points of the median nerve,and the sensory conduction velocity(SCV)and sensory nerve action potential(SNAP)of the middle finger,index finger and thumb of the median nerve in the observation group were significantly lower than those in the control group(P<0.05).The high-frequency ultrasound scans showed that the normal median nerve cross sectional area in healthy people was relatively small,and the median nerve wrist thickened and cross sectional area increased significantly in patients with CTS.Conclusion High-frequency ultrasonography and electromyography can quantitatively analyze the morphology and function of CTS,the findings of which can be used as the basis for clinical diagnosis and treatment.
