摘要
目的:通过Meta分析的方法对穴位注射治疗慢性萎缩性胃炎(CAG)的疗效进行评价,为临床应用提供循证依据。方法:检索知网数据库、万方数据库、维普数据库、中国生物医学文献数据库(China Biology Medicine disc,CBMdisc)、PubMed、Web of Science、Cochrane Library、EMbase等八个国内外数据库,检索已公开发表的关于穴位注射治疗慢性萎缩性胃炎的随机对照试验(RCT),检索时间均为建库至2022年2月。由两名研究者独立阅读、筛选文献,提取资料并根据Cochrane偏倚风险评估工具对纳入文献进行质量评价。采用RevMan 5.4软件进行Meta分析。结局指标包括临床症状有效率、临床症状治愈率、胃镜检查有效率、胃镜检查治愈率、病检有效率、临床综合疗效。结果:共纳入13项随机对照试验,样本总量964例,包括治疗组502例、对照组462例。Meta分析显示,与对照组相比,治疗组治疗CAG的临床有效率更高[RR=1.30,95%CI=(1.19,1.43),Z=5.77,P<0.00001],临床治愈率更高[RR=2.34,95%CI=(1.43,3.85),Z=3.36,P=0.0008],胃镜检查有效率更高[RR=1.22,95%CI=(1.12,1.32),Z=4.82,P<0.00001],胃镜检查治愈率更高[RR=1.83,95%CI=(1.19,2.81),Z=2.76,P=0.006],病理检查有效率更高[RR=1.32,95%CI=(1.15,1.51),Z=4.02,P<0.00001],临床综合疗效更高[RR=1.32,95%CI=(1.16,1.50),Z=4.30,P<0.00001]。结论:穴位注射治疗慢性萎缩性胃炎疗效优于常规使用中药或西药治疗,但受纳入文献质量影响,研究结论还需更多高质量的循证医学证据进一步验证。
Objective:To evaluate the efficacy of acupoint injection in the treatment of chronic atrophic gastritis by meta analysis,and to provide evidence-based basis for clinical application.Methods:Eight domestic and foreign databases including CNKI database,WANFANG database,VIP Database,China biomedical literature database(CBMdisc),PubMed,Web of Science,Cochrane Library and EMBASE were searched,The published randomized controlled trial(RCT)of acupoint injection for chronic atrophic gastritis was searched.The search time was from the establishment of the database to February 2022.Two researchers independently read and screened the literature,extracted the data,and evaluated the quality of the included literature according to the Cochrane bias risk assessment tool.Revman 5.4 software was used for meta-analysis.The outcome indicators were:the effective rate of clinical symptoms,the cure rate of clinical symptoms,the effective rate of gastroscopy,the cure rate of gastroscopy,the effective rate of disease examination,and the comprehensive clinical effect.Results:A total of 13 RCTs were included,including 964 cases,502 cases in the treatment group and 462 cases in the control group.The effective rate was higher in the treatment group(RR=1.0003,P=0.0003,P=0.0000),compared with that in the control group(RR=1.0003,P=1.0003,P=0.0000),the effective rate was higher in the treatment group(RR=1.0003,P=0.0003,P=0.0000),The curative rate of gastroscopy was higher[RR=1.83,95%CI=(1.19,2.81),2=2.76,P=0.006],the effective rate of pathological examination was higher[RR=1.32,95%CI=(1.15,1.51),2=4.02,P<0.00001],and the clinical comprehensive curative effect was higher[RR=1.32,95%CI=(1.16,1.50),2=4.30,P<0.00001].Conclusion:The curative effect of acupoint injection in the treatment of chronic atrophic gastritis is better than that of routine use of traditional Chinese medicine or western medicine,but affected by the quality of the included literature,the conclusion of this study needs more high-quality evidence-based medical evidence to be further verified.
