摘要
目的:基于细胞因子风暴理论探究不同临床病因、病情严重程度、病程肺炎患者T淋巴细胞亚群的表达特征。方法:于2016年5月至2021年4月期间,对湖北省黄冈市罗田县万密斋医院收治的216例肺炎患者进行观察性研究,比较不同临床病因、病情严重程度、病程患者T淋巴细胞亚群的含量。结果:不同病因肺炎患者T淋巴细胞亚群(包括CD3^(+)T淋巴细胞、CD4^(+)T淋巴细胞、CD8^(+)T淋巴细胞、CD4^(+)T淋巴细胞/CD8^(+)T淋巴细胞)的含量差异显著,其中细菌性肺炎患者T淋巴细胞亚群的含量相对较高(P<0.05)。不同病情严重程度肺炎患者T淋巴细胞亚群的含量差异显著,病情越严重的患者T淋巴细胞亚群的含量越高(P<0.05)。不同病程肺炎患者T淋巴细胞亚群的含量差异显著,病程越长的患者T淋巴细胞亚群的含量越高(P<0.05)。结论:T淋巴细胞亚群水平与肺炎的严重程度、病程、病因有关,可作为临床辅助诊断肺炎的参考指标。
Objective:To explore the expression characteristics of T lymphocyte subsets in patients with pneumonia with different clinical etiology,disease severity and course based on cytokine storm theory.Methods:From May 2016 to April 2021,216 patients with pneumonia admitted to Wanmizhai Hospital,Luotian County,Huanggang City,Hubei Province were investigated in an observational study to compare the content of T lymphocyte subsets in patients with different clinical etiology,severity and course of disease.Results:The content of T lymphocyte subsets(including CD3^(+)T lymphocytes,CD4^(+)T lymphocytes,CD8^(+)T lymphocytes,CD4^(+)T lymphocytes/CD8^(+)T lymphocytes)in patients with different etiology pneumonia was significantly different,and the content of T lymphocyte subsets in patients with bacterial pneumonia was relatively higher(P<0.05).The content of T lymphocyte subsets in patients with different disease severity was significantly different,and the more severe the disease was,the higher the content of T lymphocyte subsets was(P<0.05).The content of T lymph ocyte subsets in patients with different course of pneumonia was significantly different,and the longer the course of pneumonia,the higher the content of T lymphocyte subsets was(P<0.05).Conclusion:The level of T lymphocyte subsets is related to the severity,course a nd etiology of pneumonia,and can be used as a reference index for clinical diagnosis of pneumonia.
