摘要
目的分析CT血管造影(computed tomography angiography,CTA)用于肺动脉栓塞的诊断效果。方法回顾性分析2020年1月—2021年12月甘肃省庆阳市人民医院收治的75例疑似肺动脉栓塞患者的临床资料,均予以CTA检查,以肺动脉造影结果作为金标准,统计CTA对肺动脉栓塞的诊断效能,经容积再现、多平面重建以及最大密度投影3种技术对肺动脉造影结果为阳性患者的CTA图像开展重建,采用合适窗位观察,对比3种不同后处理技术的栓子检出占比。结果CTA检查的诊断准确度、灵敏度、特异度、阳性预测值及阴性预测值分别是96.00%、97.06%、85.71%、98.51%、75.00%。最大密度投影、多平面重建技术均能检测到肺动脉和主干全部栓子;多平面重建对于叶动脉栓子检出率、对于段及以下动脉的栓子检出率以及总检出率均高于容积再现,差异有统计学意义(P<0.05);最大密度投影对于段及以下动脉的栓子检出率和总检出率均高于容积再现,差异有统计学意义(P<0.05)。结论将CTA检查用于肺动脉栓塞有着较高的诊断效能,其中多平面重建技术能清晰显示出段及以下动脉的栓子,为临床诊断疾病、制订治疗方案和评定疗效提供重要依据。
Objective To analyze the diagnostic effect of computed tomography angiography(CTA)for pulmonary artery embolism.Methods A retrospective analysis was conducted on the clinical data of 75 patients with suspected pulmonary embolism admitted to Qingyang People's Hospital in Gansu Province from January 2020 to December 2021.CTA examinations were performed,and the diagnostic efficacy of CTA for pulmonary embolism was calculated using pulmonary angiography results as the gold standard.Three techniques,volume reproduction,multiplanar reconstruction,and maximum density projection,were used to reconstruct the CTA images of patients with positive pulmonary angiography results,and appropriate window observation was used,and compared the proportion of embolus detection among three different postprocessing techniques.Results The diagnostic accuracy,sensitivity,specificity,positive predictive value,and negative predictive value of CTA examination were 96.00%,97.06%,85.71%,98.51%,and 75.00%,respectively.Both maximum density projection and multi plane reconstruction techniques could detect all emboli in the pulmonary artery and main trunk;the detection rate of lobar artery emboli,segmental and below arterial emboli,and total detection rate were higher in multiplanar reconstruction than in volume reconstruction,and the difference was statistically significant(P<0.05);the maximum density projection has a statistically significant difference in the detection rate and total detection rate of emboli in segmental and below arteries compared to volume reproduction(P<0.05).Conclusion The use of CTA for pulmonary artery embolism has high diagnostic efficacy,and the multiplanar reconstruction technique can clearly show the emboli of segmental and subsegmental arteries,which provides an important basis for clinical diagnosis,treatment planning and evaluation of efficacy.
