4种大疱性皮肤病抗体检测结果分析

摘要 目的探讨4种大疱性皮肤病抗体:Dsg1、Dsg3、BP180和BP230抗体检测的意义。方法采用酶联免疫吸附法检测6808例患者血清中外周血Dsg1、Dsg3、BP180和BP230抗体,并进行阳性率分析及比较。结果(1)不同临床诊断阳性率比较,天疱疮>寻常天疱疮(PV)>类天疱疮>落叶型天疱疮(PF)>其他诊断。(2)4种大疱性皮肤病抗体阳性率为61.00%,其中阳性率BP(30.27%)>PV(15.98%)>PF(9.18%)>PV+BP(3.61%)>PF+BP(1.95%)。BP和PV+BP男性阳性率高于女性阳性率,PV和PF女性阳性率高于男性。(3)总阳性率随着年龄升高趋势升高。未成年人、中年人、老年人阳性率从高到低均为BP>PV>PF>PV+BP>PF+BP;青年人组阳性率从高到低为PV>PF>BP>PV+BP>PF+BP。在BP中老年人阳性率最高,在PV和PF中青年人阳性率最高,PV+BP和PF+BP在未成年组中阳性率为0.00%。(4)冬季阳性率最高64.16%。BP中冬季阳性率最高,秋季最低;PV四季无差异;PF中秋季阳性率最高,夏季最低;PV+BP春季阳性率最高;PF+BP组冬季阳性率最高。(5)不同地域阳性率东北>华北>西北>华东>西南>华南>华中。结论大疱性皮肤病不同的类型在不同临床诊断、不同性别、不同年龄段、不同季节及不同地区中有不同特征,联合4种大疱性皮肤病抗体检测可为疾病的诊断和鉴别提供重要依据。 Objective To explore the significance of four antibodies against bullous dermatosis,including Dsg1,Dsg3,BP180 and BP230 antibodies.Methods The serum of 6808 patients with peripheral blood Dsg1,Dsg3,BP180 and BP230 antibodies were detected by enzyme-linked immunosorbent assay,and the positive rates were analyzed and compared.Results(1)Comparing the positive rates of different clinical diagnoses,pemphigus>pemphigus vulgaris(PV)>pemphigoid>deciduous pemphigus(PF)>other diagnoses.(2)The positive rate of four kinds of bullous dermatosis antibodies was 61.00%,of which the positive rate was BP(30.27%)>PV(15.98%)>PF(9.18%)>PV+BP(3.61%)>PF+BP(1.95%).The positive rate of BP and PV+BP in men is higher than that in women,and the positive rate of PV and PF in women is higher than that in men.(3)The total positive rate increased with age.The positive rate of minors,middle-aged people and elderly people from high to low is BP>PV>PF>PV+BP>PF+BP;The positive rate of the youth group was PV>PF>BP>PV+BP>PF+BP from high to low.The positive rate of the elderly in BP is the highest,and that of the young in PV and PF is the highest.The positive rate of PV+BP and PF+BP in the juvenile group is 0.00%.(4)The highest positive rate was 64.16%in winter.The positive rate of BP was the highest in winter and the lowest in autumn;PV has no difference in four seasons;The positive rate of PF was the highest in autumn and the lowest in summer;The positive rate of PV+BP in spring was the highest;PF+BP group had the highest positive rate in winter.(5)The positive rates in different regions is Northeast>North>Northwest>East>Southwest>South>Central of China.Conclusion Different types of bullous dermatosis have different characteristics in different clinical diagnoses,different sexes,different age groups,different seasons and different regions.Combined detection of four kinds of bullous dermatosis antibodies can provide important basis for the diagnosis and differentiation of the disease.
