摘要
目的:分析多普勒超声测定孕晚期脐动脉血流动力学参数在胎儿窘迫诊断中的应用价值。方法:选取2020年1月—2022年1月泉州市光前医院收治的存在胎儿窘迫的500例孕妇,将其设为试验组;另选取同期在本院进行体检的正常妊娠孕妇500例,将其设为对照组。通过彩色多普勒超声诊断仪对两组脐动脉胎盘附着部、漂浮部、脐轮部的血流动力学指标[搏动指数(PI)、阻力指数(RI)、收缩期峰值流速与舒张末期流速的比值(S/D)]进行检测,观察比较检测结果。结果:试验组脐动脉胎盘附着部、漂浮部、脐轮部的S/D、RI、PI值均显著高于对照组,差异有统计学意义(P<0.05)。试验组脐轮部的S/D、RI、PI值均显著高于漂浮部和胎盘附着部,且漂浮部的S/D、RI、PI值均显著高于胎盘附着部,差异有统计学意义(P<0.05)。结论:通过检测脐动脉不同节段的血流动力学指标,能对胎儿血氧状态进行准确反映,能帮助诊断胎儿窘迫,其中以脐动脉脐轮部作为血流检测部位最佳。
Objective:To analyze the application value of umbilical artery hemodynamic parameters measured by Doppler ultrasound in the diagnosis of fetal distress in late pregnancy.Method:A total of 500 pregnant women with fetal distress admitted to Quanzhou Guangqian Hospital from January 2020 to January 2022 were selected as the experimental group;another 500 normal pregnant women who underwent physical examinations in our hospital during the same period were selected as the control group.The hemodynamic indexes[pulsatility index(PI),resistance index(RI),ratio of peak systolic velocity to end diastolic velocity(S/D)]of placental attachment part,floating part and umbilical wheel part of the two groups of umbilical artery were detected by color Doppler ultrasound diagnostic apparatus,and the results were observed and compared.Result:The S/D,RI and PI values of the placental attachment part,floating part and umbilical wheel part of the umbilical artery in the experimental group were significantly higher than those in the control group,the differences were statistically significant(P<0.05).The S/D,RI,and PI values of the umbilical wheel part in the experimental group were significantly higher than those of the floating part and the placental attachment part,and the S/D,RI,and PI values of the floating part were significantly higher than those of the placental attachment part,the differences were statistically significant(P<0.05).Conclusion:By detecting the hemodynamic indexes of different segments of umbilical artery,the fetal blood oxygen status can be accurately reflected,and fetal distress can be accurately diagnosed.Among them,the umbilical artery and the umbilical wheel part are the best blood flow detection sites.
