2021年宜昌市碘缺乏病监测结果分析

摘要 目的:了解宜昌市碘缺乏病的病情和重点人群碘营养状况,为今后碘缺乏病防治措施的调整提供科学依据。方法:按《湖北省2021年地方病防治工作计划》的要求,采用按人口比例概率抽样法在宜昌所辖13个县市区开展碘缺乏病监测工作。按东、西、南、北、中将每个监测县(区)划为5个片区,每个片区随机抽取1所小学的8~10岁儿童40人,孕妇20人。对抽样儿童和孕妇采集食用盐、尿样,监测碘含量。按省地方病工作要求每县3年检测一次儿童甲状腺,抽取西陵、伍家、猇亭区共300名儿童进行甲状腺容积测量。结果:共检测盐样3900份,盐碘中位数为24.50 mg/kg,碘盐覆盖率为98.41%,碘盐合格率为96.35%,合格碘盐食用率为94.82%。检测儿童尿样2600份,尿碘中位数209.30μg/L,处于碘营养充足水平。检测孕妇尿样1300份,尿碘中位数147.70μg/L,处于碘营养不足水平。查儿童甲状腺600人,1例肿大。结论:根据消除碘缺乏病的标准,宜昌市儿童的碘营养处于充足水平,但孕妇群体处于碘营养不足水平,建议持续加强地方病健康教育,对孕妇等重点人群开展碘营养指导。 Objective:To understand the status of iodine deficiency disease and iodine nutrition of key population in Yichang city,and to provide a scientific basis for the adjustment of iodine deficiency disease prevention measures in the future.Methods:According to the requirements of the 2021 Hubei Endemic Disease Prevention and Control Plan,iodine deficiency disease(IDD)monitoring was carried out in 13 counties and urban areas of Yichang by population proportional probability sampling method.Each monitoring county(district)was divided into 5 districts including the east,west,south,north,and central.There were 40 children aged 8 to 10 and 20 pregnant women randomly selected from one primary school in each district.Children and pregnant women were sampled for dietary salt and urine samples to detect iodine content.According to the requirements of provincial endemic disease work,children's thyroid was tested once every 3 years.A total of 300 children from Xiling,Wujia,and Xiaoting districts were extracted for thyroid volume measurement.Results:A total of 3900 salt samples were detected.The median of salt iodine was 24.50 mg/kg.The coverage rate of iodized salt was 98.41%.The qualified rate of iodized salt was 96.35%.The edible rate of qualified iodized salt was 94.82%.A total of 2600 urine samples were collected,with median of 209.30μg/L for urine iodine,which was at the iodine sufficient level.There were 1300 urine samples from pregnant women detected,with median of 147.70μg/L for urinary iodine,which was at iodine deficiency level.Thyroid glands were measured in 600 children,including 1 case of enlargement.Conclusion:According to the standards for the elimination of IDD,iodine nutrition for children is at an adequate level,while that for pregnant women is at an insufficient level.It is suggested that health education on endemic diseases should be continuously strengthened and iodine nutrition guidance should be provided to pregnant women and other key groups.
