熔解曲线检测结核分枝杆菌对异烟肼、利福平及乙胺丁醇的耐药性应用评估

Application Evaluation of Mycobacterium Tuberculosis Resistance to Isoniazid,Rifampicin,and Ethambutol Detected by Melting Curve

摘要 目的探讨荧光聚合酶链反应熔解曲线(简称熔解曲线)检测结核分枝杆菌(MTB)对异烟肼、利福平及乙胺丁醇的耐药性应用评估价值。方法收集2021年11月至2022年10月开封市结核病防治所收治的300例临床涂片抗酸染色阳性患者的痰标本,采用熔解曲线检测MTB对异烟肼、利福平及乙胺丁醇的耐药性,以传统罗氏药敏为金标准,评估熔解曲线检测耐药性的价值。结果300例菌株有290例为MTB;药敏检测异烟肼耐药84例(28.97%)、利福平耐药62例(21.38%)、乙胺丁醇耐药41例(14.14%);溶解曲线检测异烟肼的敏感度为83.33%、特异度为91.26%、一致率为88.97%;溶解曲线检测利福平的敏感度为91.94%、特异度为95.61%、一致率为94.83%;溶解曲线检测乙胺丁醇的敏感度为90.24%、特异度为95.98%、一致率为95.17%。结论熔解曲线检测MTB对异烟肼、利福平及乙胺丁醇的耐药性具有良好的敏感度、特异度,利于辅助MTB诊断筛查,为医师用药提供依据。 Objective To investigate the application value of fluorescence polymerase chain reaction melting curve(melting curve)in the evaluation of resistance of Mycobacterium tuberculosis(MTB)to isoniazid,rifampin,and ethambutol.Methods Sputum samples of 300 patients with acid-fast staining were collected from Kaifeng Tuberculosis Prevention and Control Institute from November 2021 to October 2022.The resistance of MTB to isoniazid,rifampicin,and ethambutol was detected by the melting curve.The value of detection of resistance by melting curve was evaluated using traditional Roche drug-sensitive as the gold standard.Results Among 300 isolates,290 were MTB.Eighty-four patients(28.97%)were resistant to isoniazid,62 patients(21.38%)were resistant to rifampicin,and 41 patients(14.14%)were resistant to ethambutol.The sensitivity,specificity,and consistency of isoniazid were 83.33%,91.26%,and 88.97%respectively.The sensitivity,specificity,and consistency of rifampicin were 91.94%,95.61%,and 94.83%respectively.The sensitivity,specificity,and consistency of ethambutol were 90.24%,95.98%,and 95.17%respectively.Conclusion The melting curve has a good sensitivity and specificity for detecting the resistance of MTB to isoniazid,rifampin,and ethambutol,which is helpful for the diagnosis and screening of MTB and provides a basis for doctors to formulate drugs.
