摘要
目的 探讨MRI检查对浸润型和腔内生长型早期宫颈癌的评估价值.方法 选取80例宫颈癌患者,均接受MRI和病理检查,分别在T2加权成像(T2WI)、动态对比增强磁共振成像(DCE-MRI)、弥散加权成像(DWI)三种序列图像上测量宫颈间质浸润最大深度,并比较MRI检查测量浸润型(深层浸润、双向浸润)和腔内生长型宫颈癌的结果与病理结果的一致性,比较T2WI、DCE-MRI、DWI三种MRI扫描序列测量宫颈间质浸润最大深度与病理结果的差值.结果 病理检查结果显示,浸润型58例,其中深层浸润40例,双向浸润18例;腔内生长型22例.MRI检查结果显示,浸润型56例,其中深层浸润40例,双向浸润16例;腔内生长型21例,漏诊3例.MRI检查T2WI、DCE-MRI、DWI三种序列测量宫颈间质浸润最大深度的结果与病理结果比较,差异无统计学意义(P﹥0.05).MRI检查T2WI、DCE-MRI、DWI三种扫描序列测量深层浸润和双向浸润宫颈癌的结果与病理检查结果的一致性较好,测量腔内生长型的一致性差.MRI检查T2WI、DCE-MRI、DWI三种扫描序列测量浸润型和腔内生长型宫颈癌宫颈间质浸润最大深度与病理结果差值比较,差异均无统计学意义(P﹥0.05).结论 早期宫颈癌患者采用MRI检查,可在一定程度上提高浸润型早期宫颈癌的诊断准确度,尽早明确宫颈癌的间质浸润深度,且MRI检查对浸润型早期宫颈癌的评估效果明显优于腔内生长型.
Objective To explore the evaluation value of MRI examination for infiltrating and intracavitary growth type early cervical cancer.Method A total of 80 patients with cervical cancer were selected and examined by MRI to measure the maximum depth of cervical interstitial infiltration on T2-weighted imaging(T2WI),dynamic contrast enhanced magnetic resonance imaging(DCE-MRI)and diffusion-weighted imaging(DWI).The MRI examination results of infiltrating(deep infiltration,bilateral infiltration)and intracavitary growth cervical cancer were compared with the pathological results.And the differences between the maximum depth of cervical interstitial infiltration measured by T2WI,DCE-MRI,and DWI-MRI scanning sequences and pathological results were also compared.Result Pathological results showed there were 58 cases of infiltrating type(40 cases of deep infiltration,18 cases of bidirectional infiltration)and 22 cases of intracavitary growth type;MRI results showed there were 56 cases of infiltrating type(40 cases of deep infiltration,16 cases of bidirectional infiltration),21 cases of intracavitary growth types,3 cases of missed diagnosis.There were no statistically significant differences between the maximum depth of cervical interstitial infiltration measured by T2WI,DCE-MRI,and DWI sequence in MRI examination and pathological results(P>0.05).The results of MRI examination of T2WI,DCE-MRI,and DWI scanning sequences for measuring deep infiltration and bidirectional infiltration of cervical cancer have good consistency with pathological examination results,but the consistency of measuring intracavitary growth type is poor.The results of maximum depth of cervical interstitial infiltration in infiltrating and intracavitary growth type cervical cancer measured by three scanning sequences of T2WI,DCE-MRI,and DWI were compared with pathological results,and there were no statistically significant differences(P>0.05).Conclusion Early cervical cancer patients using MRI examination can improve the diagnostic accuracy of infiltrating early cervical cancer to a certain extent,clarify the interstitial infiltration depth of cervical cancer as soon as possible,and MRI examination is significantly better in evaluating infiltrating type early cervical cancer than intracavitary type.
