摘要
胃癌前病变(PLGC)是指胃黏膜的异型增生和肠上皮化生,属于临床疑难疾病,目前对PLGC的发病机制尚无统一、明确的认识,无针对性治疗药物。中医药对PLGC的独特认识和治疗手段为临床提供了新选择。王庆国教授认为,PLGC病机多为正虚邪实,枢机不利,临床应坚持“扶正祛邪、调枢守神”法,治以健脾和胃、益气养阴,并针对病理因素“湿热”“瘀血”予清热解毒、活血化瘀之品;同时调畅肝胆之枢、脾胃之枢,调和阴阳,形神同治。特别强调中西医结合,注重未病先防,既病防变。
Precancerous lesion of gastric carcinoma(PLGC)refers to the dysplasia of the gastric mucosa and intestinal metaplasia,which is a clinically difficult disease.Currently,there is no unified and clear understanding of the pathogenesis of PLGC,so there is no targeted drug for treatment.The unique understanding and treatment methods of TCM for PLGC provide new options for clinical use.Professor Wang Qingguo believes that the pathogenesis of PLGC is mostly due to declined healthy qi and fierce pathogenic factors,with unfavorable pivotal mechanism.In clinical practice,it is necessary to adhere to the method of“reinforcing the healthy qi to eliminate pathogenic factors,regulating the pivot to guard the spirit”,treat it with the methods of strengthening the spleen and stomach,supplementing qi and nourishing yin,and provide medicines that can clear heat,detoxify,and promote blood circulation and remove stasis based on pathological factors such as“damp heat”and“blood stasis”;at the same time,the pivot of the liver and gallbladder should be regulated,the pivot of the spleen and stomach should be regulated,and yin and yang should be regulated,simultaneously treating the body and spirit together.In addition,Professor Wang also places special emphasis on the integration of traditional Chinese and Western medicine,emphasizing the prevention of diseases before they occur,and the prevention of changes in existing diseases.
