深髓静脉评分和脑小血管疾病患者认知功能障碍相关性研究 被引量:3

Association of deep medullary veins score with cognitive impairment in patients with cerebral small vessel disease

摘要 目的探讨深髓静脉(DMV)评分对脑小血管疾病患者认知功能障碍的相关性。方法本次研究收集我院116例患者的临床及影像学资料。所有入组研究对象均完成简易精神状态检查量表(MMSE),测试后分为认知功能障碍组(评分<27分)和认知功能正常组(≥27分)。在磁敏感图像(SWI)上根据两侧额叶、顶叶和枕叶的DMV可视情况分别给与0~3分评分,DMV评分为6个区域的评分总和(0~18分)。在磁共振液体衰减反转恢复(FLAIR)序列图像上进行侧脑室旁高信号(PWMH)Fazekas评分。结果33例MMSE评分小于27分(认知障碍组),83例评分大于等于27分(正常组)。认知功能障碍组的平均年龄(69±10)岁,高血压24例(72.7%),DMV评分中位数10(6,13)。而正常组平均年龄(57±9)岁,高血压43例(51.8%),DMV评分中位数2(0,5)。组间差异具有统计学意义(P<0.001,P=0.040,P<0.001)。DMV评分和PWMH Fazekas评分正相关(r=0.675,P<0.001),PWMH Fazekas评分和MMSE评分负相关(r=-0.689,P<0.001)。二元Logistic回归分析后,DMV评分(P<0.001)、年龄(P<0.001)与存在认知功能障碍独立相关。结论DMV评分和脑小血管疾病患者认知功能障碍相关。 Objective To investigate the correlation between the deep medullary veins(DMVs)score and cognitive impairment in patients with cerebral small vessel disease.Methods 116 patients completed the Mini-Mental State Examination(MMSE)and were divided into cognitive impairment group and normal cognitive function group according to the MMSE scores.The DMVs were divided into left and right frontal,parietal and occipital lobes on susceptibility-weighted imaging(SWI)and each region was scored 0 to 3 based on the visual condition of the vessels.The DMV score was the sum of the 6 regions(0-18).The Fazekas scores of periventricular white matter hyperintensities were determined on fluid attenuated inversion recovery(FLAIR)-weighted images.Results The MMSE scores were<27 in the cognitive impairment group(33 patients)and≥27 in the normal group(83).The patients in the cognitive impairment group were significantly older[P<0.001;mean age:(69±10)years],more commonly hypertensive(P=0.040;24,72.7%),and had higher(P<0.001)median DMV score of 10(6,13)than that of the normal group[(57±9)years;43,51.8%]with median DMV score of 2(0,5).The DMV score was correlated with Fazekas scores of periventricular white matter hyperintensities(r=0.675,P<0.001),which correlated inversely with the MMSE score(r=-0.689,P<0.001).Binary logistic regression analysis showed that the cognitive impairment was independently associated with age(P<0.001)and DMV score(P<0.001).Conclusion The DMV score is associated with cognitive impairment in patients with cerebral small vessel disease.
