摘要
目的评价应用达格列净治疗后射血分数保留心力衰竭(HFPEF)合并2型糖尿病(T2DM)患者的炎症因子及心功能指标水平。方法将佳木斯市中心医院2021年3月至2022年1月收治的86例HFPEF合并T2DM患者纳入本研究,纳入患者随机分成观察组与对照组。对照组纳入的43例患者应用阿卡泼糖、二甲双胍等常规治疗。观察组纳入的43例患者基于对照组基础上应用达格列净治疗。观察两组患者的总有效率、不良反应发生情况;比较治疗前后白细胞介素-6(IL-6)、C反应蛋白(CRP)、肿瘤坏死因子-α(TNF-α)、左室舒张末期内径(LVEDD)、左室射血分数(LVEF)水平。结果与对照组相比,观察组总有效率更高(76.64%比95.35%,P<0.05)。治疗后观察组和对照组患者LVEF呈现增高趋势,LVEDD、BNP、IL-6、CRP、TNF-α水平下降,观察组上述指标较对照组更优(P<0.05)。两组总不良反应发生率相比差异无统计学意义(P>0.05)。结论达格列净治疗HFPEF合并T2DM患者有较好的临床效果,可改善患者的心功能,有效抑制炎症,且不增加不良反应,安全有效。
Objective To evaluate the inflammatory factors and cardiac function indicators in patients with type 2 diabetes treated with dagliazin after mellitus(T2DM)heart failure with preserved ejection fraction(HFPEF).Methods From March 2021 to January 2022,86 patients with T2DM and HFPEF treated in Jiamusi Central Hospital were selected as the subjects of this exploration,and the selected patients were divided into observation group and control group according to the random number table method,with 43 cases in each group.The control group was treated with conventional therapy such as acaplose and metformin.The observation group was treated with dagliazin based on the control group.The total effective rate and the occurrence of adverse reactions of the two groups were observed.The interleukin-6(IL-6),C-reactive protein(CRP),tumor necrosis factor-α(TNF-α),left ventricular end diastolic diameter(LVEDD)and left ventricular ejection fraction(LVEF)of the two groups before and after treatment were compared.Results Compared with the control group,the total effective rate of the observation group was higher(76.64%vs.95.35%,P<0.05).After treatment,LVEF of the two groups showed an increasing trend,while LVEDD,BNP,IL-6,CRP and TNF-αshowed a decreasing trend,and the observation group was better than the control group(P<0.05).The incidence of total adverse effects in the two groups was not statistically significant(P>0.05).Conclusion The treatment of daglizin in HFPEF combined with T2DM patients has good clinical efficacy,which can improve the cardiac function of patients,effectively inhibit inflammation,without increasing adverse reactions,and be safe and effective in clinical application.
