阴道分泌物细菌周长数量诊断细菌性阴道炎的应用价值

摘要 目的:探讨阴道分泌物细菌周长数量诊断细菌性阴道炎的应用价值。方法:选取2022年12月—2023年2月在长沙市中心医院进行阴道分泌物检查的412例患者作为研究对象,均进行细菌周长的测量及数量统计,分析不同周长细菌的数量诊断细菌性阴道炎的效能。结果:细菌性阴道炎患者240例,非细菌性阴道炎患者172例。412份阴道分泌物标本中共检测出48种周长细菌,其中周长最长为60.7µm,共1例,最短为0.7µm,共33例。周长为4.2µm的细菌数量最多,共351例。细菌性阴道炎组6.3、4.2、5.6、3.5、4.9、7.0、7.7、9.1µm的细菌数量多于非细菌性阴道炎组,差异有统计学意义(P<0.001)。Pearson相关性分析显示,周长4.9µm与周长5.6µm的细菌相关性最高(r=0.935,P<0.001),周长1.4µm与周长14µm的细菌相关性最低(r=0.098,P<0.048)。受试者操作特征(ROC)曲线显示,周长6.3µm细菌数量的ROC曲线下总面积(AUC)最大,为0.886(P<0.001);AUC>0.5,且P<0.001的有28种,周长3.5、4.2、4.9、5.6、6.3µm的细菌数量诊断细菌性阴道炎的敏感性分别为81.7%、76.7%、77.9%、78.8%、71.3%,特异性分别为83.7%、90.7%、82.0%、83.1%、89.0%。结论:阴道分泌物细菌周长分布具有一定规律性,周长为3.5、4.2、4.9、5.6、6.3µm等细菌的数量是潜在的细菌性阴道炎诊断指标。 Objective:To explore the application value of bacterial circumference and count of vaginal secretion in the diagnosis of bacterial vaginitis.Methods:A total of 412 patients undergoing vaginal secretion test in Changsha Central Hospital from December 2022 to February 2023 were selected as the study subjects.All patients received the measurement of bacterial circumference and the count statistics.The efficacy of the counts of the bacteria with different circumferences in the diagnosis of bacterial vaginitis was analyzed.Results:There were 240 patients with bacterial vaginitis and 172 patients with non-bacterial vaginitis.A total of 48 bacterial circumferences were detected in 412 vaginal secretion samples,of which the longest circumference was 60.7µm in a total of 1 case,and the shortest circumference was 0.7µm in a total of 33 cases.The bacteria with a circumference of 4.2µm had the highest count,in a total of 351 cases.The counts of bacteria with a circumference of 6.3µm,4.2µm,5.6µm,3.5µm,4.9µm,7.0µm,7.7µm,and 9.1µm in the bacterial vaginitis group were higher than those in the non-bacterial vaginitis group,and the difference was statistically significant(P<0.001).Pearson correlation analysis showed that,bacteria with a circumference of 4.9µm and 5.6µm had the highest correlation(r=0.935,P<0.001),and bacteria with a circumference of 1.4µm and 14µm had the lowest correlation(r=0.098,P<0.048).The Receiver Operating Characteristic(ROC)curve showed that,the total area under the ROC curve(AUC)for the count of bacteria with a circumference of 6.3µm was the largest,which was 0.886(P<0.001).There were 28 kinds of bacterial circumferences with AUC>0.5 and P<0.001.The sensitivity of the counts of bacteria with a circumference of 3.5µm,4.2µm,4.9µm,5.6µm and 6.3µm in the diagnosis of bacterial vaginitis was 81.7%,76.7%,77.9%,78.8%and 71.3%,and the specificity was 83.7%,90.7%,82.0%,83.1%and 89.0%,respectively.Conclusion:The distribution of bacterial circumference of vaginal secretion has certain regularity.The counts of bacteria with a circumference of 3.5µm,4.2µm,4.9µm,5.6µm and 6.3µm are potential diagnostic indicators of bacterial vaginitis.
