肌力训练捆绑节律化听觉注意刺激结合平衡垫训练对帕金森病患者下肢功能及认知功能的影响 被引量:1

摘要 目的探讨肌力训练捆绑节律化听觉注意刺激结合平衡垫训练对帕金森病患者下肢功能及认知功能的影响。方法选取2019年7月至2021年7月我科收治的80例帕金森病患者作为研究对象,随机将其分为对照组与观察组,各40例。对照组采用常规康复护理和平衡垫训练,观察组在对照组基础上给予肌力训练捆绑节律化听觉注意刺激。比较两组的干预效果。结果干预后,观察组的帕金森病评分量表Ⅲ评分高于对照组,单腿睁眼站立时间、单腿闭眼站立时间长于对照组(P<0.05)。干预后,观察组的Fugl-Meyer运动功能量表下肢部分(FMA-LE)评分高于对照组,6 min步行试验(6MWT)长于对照组(P<0.05)。干预后,观察组的同型半胱氨酸(Hcy)、胱抑素C(CysC)、帕金森病蛋白7(PAPK7)水平低于对照组,神经营养因子-3(NT-3)水平高于对照组(P<0.05)。干预后,观察组的蒙特利尔认知评估量表(MoCA)、日常生活活动能力量表(ADL)评分均高于对照组(P<0.05)。结论肌力训练捆绑节律化听觉注意刺激结合平衡垫训练干预有助于促进帕金森病患者步态姿势、平衡功能、下肢功能改善,也能调节神经相关因子水平,促进认知功能改善,提高日常生活能力。 Objective To explore the effects of muscle strength training bundled rhythmic auditory attention stimulation combined with balance pad training on lower limb function and cognitive function in patients with Parkinson's disease.Methods A total of 80 patients with Parkinson's disease admitted in our department from July 2019 to July 2021 were selected as study objects and randomly divided into control group and observation group,with 40 cases in each group.The control group was given routine rehabilitation nursing and balance pad training,while the observation group was given muscle strength training bundled rhythmic auditory attention stimulation on the basis of the control group.The intervention effects of the two groups were compared.Results After intervention,the Parkinson's disease score scaleⅢscore of the observation group was higher than that of the control group,and the standing time of one leg with eyes open and one leg with eyes closed were longer that those of the control group(P<0.05).After intervention,the Lower Extremity Portion of the Fugl-Meyer Motor Assessment(FMA-LE)score of the observation group was higher than that of the control group,and the 6 min walk test(6MWT)was longer than that of the control group(P<0.05).After intervention,the levels of homocysteine(Hcy),cystatin C(CysC)and Parkinson's disease protein 7(PAPK7)in the observation group were lower than those in the control group,and the level of neurotrophic factor-3(NT-3)was higher than that in the control group(P<0.05).After intervention,the scores of Montreal Cognitive Assessment Scale(MoCA)and Activity of Daily Living Scale(ADL)in the observation group were higher than those in the control group(P<0.05).Conclusion Muscle strength training bundled rhythmic auditory attention stimulation combined with balance pad training intervention is helpful to promote the improvement of gait posture,balance function and lower limb function in patients with Parkinson's disease,it can also regulate the levels of nerve-related factors,promote the improvement of cognitive function and improve the ability of daily life.
