摘要
目的了解接受嵌合抗原受体T细胞(CAR-T)治疗的晚期卵巢癌患者的真实体验,为采取有针对性的护理干预、提升患者生活质量提供参考。方法采用描述性现象学研究方法,选取2021年1-9月在同济大学附属东方医院接受CAR-T治疗的晚期卵巢癌患者进行半结构式访谈,并采用Colaizzi分析程序对资料进行分析、整理并提炼主题。结果本次研究共访谈接受CAR-T治疗的晚期卵巢癌患者8例,其在参加临床试验的不同阶段(决定治疗前、治疗期间及出院随访期)体现出不同的心理特征,具体可归纳为:参与CAR-T临床试验全程充满矛盾心理、家庭的支持及医护人员的悉心照护是患者决定和坚持治疗的重要动力来源、治疗期间希望得到更多信息和指导。结论医护人员和家属应密切关注接受CAR-T临床试验晚期卵巢癌患者的心理、生理状况及社会支持,并通过规范化管理和针对性的知识宣教,促进患者满意度和生活质量提升。
Objective To understand the real experience of patients with advanced ovarian cancer treated by chimeric antigen receptor T cells(CAR-T),providing a reference for taking targeted nursing intervention and improving patients'quality of life.Methods Patients with advanced ovarian cancer who received CAR-T therapy at Shanghai East Hospital Affiliated to Tongji University from January to September 2021 were selected.They underwent semi-structured interviews by descriptive phenomenological research method,and their interview data were analyzed,sorted out and refined by Colaizzi's method of data analysis.Results A total of 8 patients with advanced ovarian cancer who received CAR-T therapy were interviewed this time.They showed different psychological characteristics at different stages of clinical trials(before deciding to participate,during treatment and during discharge follow-up),which can be summarized as follows:Patients are full of ambivalence during CAR-T clinical trials;family support and careful care of healthcare providers are important motivators for patients to decide and insist on treatment;patients hope to get more information and guidance during treatment.Conclusion Healthcare providers and patient families should pay close attention to the psychological status,physiological status and social support of patients with advanced ovarian cancer undergoing CAR-T clinical trials,and promote their satisfaction and quality of life through standardized management and targeted knowledge education.
