摘要 目的 探究便携式呼吸机在院内转运中氧气配备的推算公式,为转运危重患者配备氧气源提供依据。方法 模拟转运过程,将Oxylog3000plus便携式呼吸机连接至模拟肺,在转运呼吸机氧气进气管串接气体质量流量计,记录转运中各个环节的氧气消耗值,并分析呼吸机运行过程中氧气消耗实测值与理想计算值、修正计算值和呼吸机监测值的相关性。结果 呼吸机运行阶段,氧气消耗的实测值、理想计算值、修正计算值和呼吸机监测值比较,差异均有统计学意义(P<0.001);测试呼吸机管路与模拟肺断开连接30 s,呼吸末正压为0、5、10 cmH_(2)O时断开连接过程中氧气消耗量比较,差异均有统计学意义(P<0.001)。Oxylog3000plus便携式呼吸机用于院内转运时氧气配备的推算公式为:转运呼吸机配备氧气源总量=氧气消耗的修正计算值/min×(估计转运时间+试用时间+预留时间)+断开连接30 s氧气消耗量+氧气钢瓶最低供气压力对应的残余氧气量。结论 便携式呼吸机在院内转运中氧气配备的推算公式可为危重患者院内转运配备合适氧气源、确保转运安全、减少氧气浪费提供一定的参考依据。 Objective To explore the calculation formula for oxygen supply of portable ventilators during intra-hospital transportation,and provide a basis for oxygen supply for critical patients during transportation.Methods The transportation process was simulated by connecting the Oxylog3000plus portable ventilator to a simulated lung,and a gas mass flow meter was connected to the ventilator’s oxygen inlet pipe to record the oxygen consumption at each stage of transportation.The correlation between the measured oxygen consumption during the ventilator’s operation and the ideal calculated value,modified calculated value,and monitored value were analyzed.Results During the operation of the ventilator,the measured oxygen consumption,the difference between the measured value,ideal calculated value,corrected calculated value,and ventilator monitoring value of oxygen consumption was statistically significant(P<0.001).When the ventilator circuit was disconnected from the simulated lung for 30 s,there were statistically significant difference in oxygen consumption during disconnection when the positive end respiratory pressure was 0,5,and 10 cm H_(2)O(P<0.001).The calculation formula for oxygen supply of Oxylog3000plus portable ventilator during intrahospital transportation was:total oxygen supply of the ventilator=corrected calculated oxygen consumption value/min×(estimated transportation time+trial time+reserve time)+oxygen consumption during 30 s disconnection+residual oxygen volume corresponding to the minimum gas supply pressure of the oxygen cylinder.Conclusion The calculation formula for oxygen allocation during the transportation of portable ventilators in hospitals can provide a certain reference basis for equipping critically ill patients with appropriate oxygen sources,ensuring transportation safety,and reducing oxygen waste.
