摘要
目的分析低剂量纳美芬在乙醇与安眠药混合中毒患者中的应用。方法2017年1月至2019年12月南昌市第一医院接收的87例乙醇与安眠药混合中毒患者,根据药物使用剂量不同分组,分为A组(47例)及B组(40例)。两组均接受常规治疗,在此基础上A组采用0.5μg/kg纳美芬治疗,B组采用1.0 g/kg纳美芬治疗。比较两组临床疗效、清醒时间及症状消失时间,比较两组治疗前、停药后肝功能指标水平及不良反应发生情况。结果A组临床疗效总有效率为91.49%,B组为92.50%,两组临床疗效比较差异无统计学意义(χ^(2)=0.03,P=0.863);两组清醒时间与症状消失时间比较差异无统计学意义(t=0.21、0.59,P=0.834、0.558);停药后,两组肝功能各指标较治疗前明显降低(P<0.05),A组总胆红素(TBil)、γ-谷氨酸转肽酶(γ-GT)、天门冬酸氨基转移酶(AST)、丙氨酸氨基转移酶(ALT)水平均明显低于B组(P<0.05);治疗期间,两组精神萎靡等不良反应发生差异无统计学意义(P>0.05)。结论0.5μg/kg与1.0μg/kg两种剂量盐酸纳美芬注射液在乙醇与安眠药混合中毒患者中均效果显著,且0.5μg/kg盐酸纳美芬注射液更利于保护肝功能。
Objective To investigate the application of low-dose nalmefene in patients with alcohol and sleeping pills mixed poisoning.Methods 87 patients with alcohol and sleeping pills mixed poisoning who were admitted to the hospital from January 2017 to December 2019 were divided into group A(47 cases)and group B(40 cases)according to the dose of nalmefene.Both groups underwent conventional treatments,on this basis,group A was treated with 0.5μg/kg of nalmefene,while group B was treated with 1.0μg/kg of nalmefene.Clinical efficacy,waking hours,disappearance time of symptoms,the levels of liver function indicators,and adverse reactions were compared between the two groups.Results After treatment,the total clinical response rates in group A and group B were 91.49%and 92.50%,showing no significant difference between the groups(χ^(2)=0.03,P=0.863).The waking hours and disappearance time of symptoms were similar in the two groups(t=0.21,0.59,P=0.834,0.558).After drug withdrawal,liver function indicators in the two groups decreased(P<0.05).The levels of total bilirubin(TBil),gammaglutamyltransferase(γ-GT),aspartate aminotransferase(AST)and alanine aminotransferase(ALT)in group A were significantly lower than those in group B(P<0.05).The incidence of adverse reactions showed no significant difference between the two groups(P>0.05).Conclusion Both of 0.5μg/kg and 1.0μg/kg of nalmefene are effective in the treatment of patients with alcohol and sleeping pills mixed poisoning.However,0.5μg/kg of nalmefene is more beneficial to protect liver function.
