摘要
目的:探讨妊娠期糖尿病(gestational diabetes mellitus,GDM)患者并发甲状腺功能减退症(简称甲减)的相关影响因素。方法:选择2020年1月—2022年8月曹县人民医院产科收治的76例GDM并发甲减患者为研究组,并选择同时期曹县人民医院产科收治的80例GDM患者为对照组。经自制调查量表分析两组临床资料,并经单因素、多因素logistic分析影响因素,统计两组妊娠结局。结果:两组年龄、甲状腺疾病史、甲状腺疾病家族史、妊娠期高血压史、凝血酶时间(thrombin time,TT)、凝血酶原时间(prothrombin time,PT)比较,差异有统计学意义(P<0.05);多因素logistic分析结果显示,年龄≥30岁、甲状腺疾病史、甲状腺疾病家族史、妊娠期高血压史、TT>18 s、PT>13.0 s均是GDM并发甲减的主要影响因素(P<0.05);研究组早产、胎膜早破、新生儿低血糖、胎儿窘迫发生率较对照组高,差异有统计学意义(P<0.05)。结论:GDM患者于妊娠期间受年龄较大、有甲状腺疾病史、甲状腺疾病家族史、妊娠期高血压及凝血功能异常等相关因素影响,易并发甲减,从而发生早产、新生儿低血糖、流产等不良妊娠结局。
Objective:To explore the related influencing factors of hypothyroidism in patients with gestational diabetes(GDM).Method:From January 2020 to August 2022,76 patients with GDM complicated with hypothyroidism who admitted in the Obstetrics Department of Cao County People's Hospital were selected as the study group,and 80 patients with GDM who admitted in the Obstetrics Department of Cao County People's Hospital in the same period were selected as the control group.The clinical data of two groups were analyzed by self-made survey scale,and the influencing factors of GDM complicated with hypothyroidism were analyzed by single factor and multi factor logistic analysis,and the pregnancy outcomes of two groups were counted.Result:There were significant differences in the age,thyroid disease history,thyroid disease family history,pregnancy hypertension history,thrombin time(TT),prothrombin time(PT)between the two groups(P<0.05);multivariate logistic analysis showed that age≥30 years old,thyroid disease history,thyroid disease family history,pregnancy hypertension history,TT>18 s,PT>13.0 s were the main influencing factors of GDM complicated with hypothyroidism(P<0.05);the incidence of premature delivery,premature rupture of membranes,neonatal hypoglycemia and fetal distress in the study group were significantly higher than those in the control group,the differences were statistically significant(P<0.05).Conclusion:GDM patients are susceptible to hypothyroidism during pregnancy due to factors such as older age,thyroid disease history,thyroid disease family history,gestational hypertension,and abnormal coagulation function,it is easy to be complicated with hypothyroidism,as a result,adverse pregnancy outcomes such as premature delivery,neonatal hypoglycemia and abortion occur.
