肺表面活性物质联合布地奈德气管内滴入预防早产儿支气管肺发育不良的效果 被引量:2

Pulmonary Surfactant Combined with Budesonide Endotracheal Drip in the Prevent of Bronchopulmonary Dysplasia in Premature Infants

摘要 目的:研究肺表面活性物质(PS)联合布地奈德气管内滴入对早产儿支气管肺发育不良(BPD)的预防效果。方法:选择2020年1月—2021年1月桂林市妇幼保健院收治的80例呼吸窘迫综合征(RDS)早产儿,以随机数表法将其分为试验组、对照组,各40例。对照组行PS气管内滴入治疗,试验组在对照组基础上行布地奈德气管内滴入治疗。比较两组用药前后血气指标,BPD发生率、相关时间指标及治疗结果。结果:用药24 h后,试验组动脉血气pH值、氧分压(PaO_(2))水平高于对照组,二氧化碳分压(PaCO_(2))水平低于对照组,差异有统计学意义(P<0.05)。试验组BPD发生率低于对照组,差异有统计学意义(P<0.05)。试验组呼吸机使用总时间、氧气依赖总时间、新生儿重症监护室入住时间短于对照组,差异有统计学意义(P<0.05)。试验组PS重复使用比例、PS使用次数优于对照组,差异有统计学意义(P<0.05)。两组并发症发生率比较,差异无统计学意义(P>0.05)。结论:PS联合布地奈德气管内滴入治疗可有效预防早产儿BPD的发生,同时对早产儿出生后的氧合及呼吸机使用时间亦有积极的改善效果,整体安全性未受影响。 Objective:To study the preventive effects of pulmonary surfactant(PS)combined with Budesonide endotracheal drip on bronchopulmonary dysplasia(BPD)in premature infants.Method:A total of 80 premature infants with respiratory distress syndrome(RDS)admitted to Guilin Maternal and Child Health Hospital from January 2020 to January 2021 were selected,they were randomly divided into experimental group and control group according to random number table,with 40 cases in each group.The control group received PS endotracheal drip treatment,while the experimental group received Budesonide endotracheal drip treatment on the basis of the control group.Compared the blood gas indicators before and after medication,incidence of BPD,relevant time indicators,and treatment outcomes between the two groups.Result:After 24 h of medication,the arterial blood gas pH value and partial pressure of oxygen(PaO_(2))level in the experimental group were higher than those in the control group,while the partial pressure of carbon dioxide(PaCO_(2))level was lower than that in the control group,the differences were statistically significant(P<0.05).The incidence of BPD in the experimental group was lower than that in the control group,and the difference was statistically significant(P<0.05).The total time of ventilator use,oxygen dependence,and neonatal intensive care unit stay in the experimental group were shorter than those in the control group,the differences were statistically significant(P<0.05).The proportion and frequency of PS reuse in the experimental group were better than those in the control group,the differences were statistically significant(P<0.05).There was no statistically significant difference in the incidence of complications between the two groups(P>0.05).Conclusion:The combination of PS and Budesonide endotracheal drip therapy can effectively prevent the occurrence of BPD in premature infants,and has a positive improvement effect on postoperative oxygenation and ventilator use time in premature infants,the overall safety is not affected.
