TCF7L2基因多态性、HbA1c、血浆致动脉粥样硬化指数与2型糖尿病合并冠心病的关系研究 被引量:3

摘要 目的探讨转录因子7类似物2(TCF7L2)基因多态性、糖化血红蛋白(HbA1c)、血浆致动脉粥样硬化指数(AIP)与2型糖尿病合并冠心病的关系。方法回顾性选取2020年11月至2022年6月在重庆大学附属江津医院治疗的2型糖尿病患者200例,其中合并冠心病患者67例(观察组),未合并冠心病患者133例(对照组)。比较观察组和对照组的临床资料、HbA1c、AIP、TCF7L2基因多态性差异,并比较观察组不同狭窄程度患者HbA1c、AIP及TCF7L2基因多态性;分析2型糖尿病合并冠心病的影响因素。结果观察组年龄、吸烟患者比率、空腹血糖、HbA1c、糖尿病病程、AIP和甘油三酯分别为(68.82±8.10)岁、49.25%、(9.28±2.12)mmol/L、(8.89±1.82)%、(12.54±2.11)年、1.01±0.32、(2.10±0.67)mmol/L,明显高于对照组[(61.19±9.40)岁、28.57%、(8.33±2.03)mmol/L、(7.50±1.60)%、(9.93±1.96)年、0.60±0.27、(1.89±0.60)mmol/L],差异均有统计学意义(P<0.05)。观察组患者rs290481位点AG+GG基因比例为67.16%,明显高于对照组(34.59%),差异有统计学意义(P<0.05)。观察组和对照组rs7903146位点多态性比较,差异无统计学意义(P>0.05)。中重度狭窄患者HbA1c和AIP分别为(10.30±1.56)%和1.30±0.26,明显高于轻度狭窄患者[(8.20±1.45)%,0.87±0.24],差异均有统计学意义(P<0.05)。中重度狭窄患者TCF7L2基因rs290481位点AG+GG基因型比例为86.36%,明显高于轻度狭窄患者(13.64%),差异有统计学意义(P<0.05)。Logistic回归分析显示:年龄、吸烟、HbA1c、糖尿病病程、AIP及TCF7L2基因rs290481位点多态性为2型糖尿病合并冠心病的影响因素(P<0.05)。结论TCF7L2基因rs290481位点多态性、HbA1c、AIP是2型糖尿病合并冠心病的影响因素,同时与患者冠状动脉狭窄程度有一定关系。 Objective To explore the relationship between transcription factor 7 analogue 2(TCF7L2)gene polymorphism,glycosylated hemoglobin(HbA1c),plasma atherogenic index(AIP)and type 2 diabetes with coronary heart disease.Methods A total of 200 patients with type 2 diabetes who were treated in Chongqing University Jiangjin Hospital from November 2020 to June 2022 were selected,including 67 patients with coronary heart disease(observation group)and 133 patients without coronary heart disease(control group).The clinical data,HbA1c,AIP,and TCF7L2 gene polymorphisms of the observation group and the control group were compared,and the HbA1c,AIP,and TCF7L2 gene polymorphisms of patients with different degrees of stenosis in the observation group were compared;the influencing factors of type 2 diabetes with coronary heart disease were analyzed.Results The age,smoking proportion,fasting blood glucose,HbA1c,course of diabetes,AIP and Triglyceride in the observation group were(68.82±8.10)years old,49.25%,(9.28±2.12)mmol/L,(8.89±1.82)%,(12.54±2.11)years,1.01±0.32 and(2.10±0.67)mmol/L,respectively,which were significantly higher than those in the control group[(61.19±9.40)years old,28.57%,(8.33±2.03)mmol/L,(7.50±1.60)%,(9.93±1.96)years,0.60±0.27,(1.89±0.60)mmol/L],the differences were statistically significant(P<0.05).The proportion of AG+GG gene at rs290481 locus of TCF7L2 gene in the observation group was 67.16%,which was significantly higher than that in the control group(34.59%),the difference was statistically significant(P<0.05).There was no statistically significant difference in rs7903146 polymorphism of TCF7L2 gene between the observation group and the control group(P>0.05).HbA1c and AIP in patients with moderate and severe stenosis were(10.30±1.56)%and 1.30±0.26,respectively,which were significantly higher than those in patients with mild stenosis[(8.20±1.45)%,0.87±0.24],the differences were statistically significant(P<0.05).The proportion of AG+GG genotype at rs290481 of TCF7L2 gene in patients with moderate and severe stenosis was 86.36%,which was significantly higher than that in patients with mild stenosis(13.64%),the difference was statistically significant(P<0.05).Logistic regression analysis showed that age,smoking,HbA1c,diabetes duration,AIP and TCF7L2 gene rs290481 polymorphism were the influencing factors of type 2 diabetes with coronary heart disease(P<0.05).Conclusion The rs290481 polymorphism of TCF7L2 gene,HbA1c,AIP are the influencing factors of type 2 diabetes with coronary heart disease,and are also related to the degree of coronary stenosis.
