健康体检人群血清25-(OH)-D3、炎性介质水平与空腹血糖受损的关联性探讨

摘要 目的探讨健康体检人群血清25羟维生素D(25-(OH)-D3)、炎性介质水平与空腹血糖受损的关联性。方法本次研究共纳入研究对象120例,均为于本院体检者,于2021年1月至2022年12月完成选例,依据空腹血糖水平高低将其分为对照组(血糖正常者,50例)、观察组(血糖受损者,70例)。比较两组血清甘油三酯(TG)、总胆固醇(TC)、低密度脂蛋白胆固醇(LDL-C)、高密度脂蛋白胆固醇(HDL-C)、糖化血红蛋白(HbA1c)、单核细胞趋化因子-1(MCP-1)、白细胞介素6(IL-6)、白细胞介素1(IL-1)、肿瘤坏死因子(TNF-α)、25-(OH)-D3水平,观察血清25-(OH)-D3、炎性介质水平与空腹血糖受损的相关性。结果与对照组相比较,观察组血清TG、TC、LDL-C、HDL-C、HbA1c、MCP-1、IL-6、IL-1、TNF-α水平更高,25-(OH)-D3水平更低,空腹血糖受损情况与血清MCP-1、IL-6、IL-1、TNF-α水平呈正相关,与血清25-(OH)-D3呈负相关,差异有统计学意义(P<0.05)。结论空腹血糖受损情况与炎性因子水平呈正相关,与血清25-(OH)-D3呈负相关,炎性因子水平越高机体血糖水平更高,25-(OH)-D3水平越低机体血糖水平越高,临床可以通过控制机体炎症及25-(OH)-D3水平等方法阻止血糖受损进展至糖尿病。 Objective To investigate the relationship between serum 25 hydroxyvitamin D(25-(OH)-D3),inflammatory mediators and impaired fasting blood glucose in healthy people.Methods A total of 120 subjects were included in this study,all of whom underwent physical examination in our hospital from January 2021 to December 2022.According to the level of fasting blood glucose,they were divided into the control group(50 cases of normal blood glucose)and the observation group(70 cases of impaired blood glucose).Serum triglyceride(TG),total cholesterol(TC),low density lipoprotein cholesterol(LDL-C),high density lipoprotein cholesterol(HDL-C),glycated hemoglobin(HbA1c),monocyte chemokine-1(MCP-1),interleukin-6(IL-6),interleukin-1(IL-1)and tumor necrosis factor(TNF)-α,25-(OH)-D3 levels were compared between the two groups.Results Compared with the control group,the serum levels of TG,TC,LDL-C,HDL-C,HbA1c,MCP-1,IL-6,IL-1 and TNF-αof the observation group were higher,and the level of 25-(OH)-D3 was lower.The impaired fasting blood glucose was positively correlated with the levels of serum MCP-1,IL-6,IL-1 and TNF-α,and negatively correlated with serum 25-(OH)-D3,with statistical significances(P<0.05).Conclusion The impaired fasting blood glucose is positively correlated with the level of inflammatory factors,and negatively correlated with the level of serum 25-(OH)-D3.The higher the level of inflammatory factors,the higher the level of blood glucose,and the lower the level of 25-(OH)-D3,the higher the level of blood glucose.The progression of impaired blood sugar to diabetes can be prevented clinically by controlling inflammation and 25-(OH)-D3 levels.
