急性心肌梗死患者经皮冠脉介入术后慢血流/无复流的影响因素分析

Analysis of Influencing Factors of Slow Blood Flow/No-Reflow in Patients with Acute Myocardial Infarction After Percutaneous Coronary Intervention

摘要 目的:分析急性心肌梗死(AMI)患者经皮冠脉介入术后慢血流/无复流的影响因素。方法:选取2019年1月—2022年4月株洲市二医院收治的353例AMI患者作为研究对象,根据患者经皮冠脉介入术后血流情况分为慢血流/无复流组(n=85)及正常血流组(n=268)。收集两组患者临床资料,通过单因素分析及多因素logistic回归分析法分析AMI患者经皮冠脉介入术后慢血流/无复流的影响因素。结果:多因素logistic分析结果显示,收缩压升高为AMI患者经皮冠脉介入术后慢血流/无复流的保护因素(OR=0.518,P<0.05);出现症状至经皮冠状动脉介入(PCI)时间6~12 h、Killip分级≥Ⅱ级、血糖(GLU)水平升高、同型半胱氨酸(Hcy)水平升高、D-二聚体(D-D)水平升高为AMI患者经皮冠脉介入术后慢血流/无复流的危险因素(OR=2.428、2.221、1.972、1.855、2.090,P<0.05)。结论:收缩压水平、出现症状至PCI时间、Killip分级、GLU水平、Hcy水平、D-D水平为AMI患者经皮冠脉介入术后慢血流/无复流的影响因素,临床可对高危患者采取针对性干预措施,以改善其预后。 Objective:To analyze the influencing factor of slow blood flow/no-reflow in patients with acute myocardial infarction(AMI)after percutaneous coronary intervention.Methods:A total of 353 patients with AMI who were admitted to Zhuzhou Second Hospital from January 2019 to April 2022 were selected as the study subjects.According to the blood flow after percutaneous coronary intervention,the patients were divided into the slow blood flow/no-reflow group(n=85)and the normal blood flow group(n=268).The clinical data of patients in two groups were collected.Univariate analysis and multivariate logistic regression analysis were used to analyze the influencing factors of slow blood flow/no-reflow in patients with AMI after percutaneous coronary intervention.Results:Multivariate logistic analysis results showed that,elevated systolic blood pressure was protective factor for slow blood flow/no-reflow in patients with AMI after percutaneous coronary intervention(OR=0.518,P<0.05).The time from symptom onset to percutaneous coronary intervention(PCI)of 6-12 h,Killip grade≥Ⅱ,elevated glucose(GLU)level,elevated homocysteine(Hcy)level,elevated D-dimer(D-D)level were influencing factors for slow blood flow/no-reflow in AMI patients after percutaneous coronary intervention(OR=2.428,2.221,1.972,1.855,2.090,P<0.05).Conclusion:The systolic blood pressure level,time from symptoms to PCI,Killip grade,GLU level,Hcy level,D-D level are the influencing factors of slow blood flow/noreflow in patients with AMI after percutaneous coronary intervention,clinical targeted intervention strategies could be taken for high-risk patients to improve their prognosis.
