摘要
目的 探讨呼吸道合胞病毒(RSV)下呼吸道感染患儿血清微小RNA-19a-3p(miR-19a-3p)和细胞因子信号传导抑制蛋白1(SOCS1)的表达及临床意义。方法 选取2019年1月—2022年1月在本院儿科接受的呼吸道感染患儿180例为研究对象。RSV-RNA结果呈阳性的患儿(80例)作为RSV感染组,将RSV-RNA结果呈阴性的患儿(100例)为对照组。根据入院时Lowell评分进一步将RSV感染组分为重度组(Lowell评分≥10分)和轻度组(Lowell评分<10分)。采用实时定量PCR检测血液中miR-19a-3p、SOCS1 mRNA水平;患儿血清中miR-19a-3p、SOCS1表达的相关性分析采用Pearson法。结果 两组中的临床表现(发热、气促、喘息、呕吐、腹泻)和影像学胸片(散在斑片状影)之间有显著性差异(P<0.05)。相较于对照组,RSV感染组患者血清中的IL-6、TNF-α、IL-1β水平显著升高(P均<0.05)。相较对照组,RSV感染组患者血清中的miR-19a-3p、SOCS1水平显著升高(P均<0.05)。相较于轻度组,RSV重度组患者血清中的miR-19a-3p、SOCS1水平显著升高(P均<0.05)。RSV患者血清miR-19a-3p与SOCS1、IL-6、TNF-α、IL-1β水平呈正相关(r=0.701、0.557、0.569、0.613,P均<0.05)。SOCS1与IL-6、TNF-α、IL-1β水平呈正相关(r=0.592、0.571、0.564,P<0.05)。ROC结果显示,RSV患者血清中miR-19a-3p、SOCS1水平及二者联合预测RSV感染严重程度的AUC分别为0.714、0.735、0.851,其中联合预测AUC显著高于二者单独预测AUC(Z_(二者联合-miR-19a-3p)=3.217,P<0.05)(Z_(二者联合-SOCS1)=3.679,P<0.05)。结论 RSV患者血液miR-19a-3p、SOCS1水平较高,二者成正相关性,且均与RSV患者的IL-6、TNF-α、IL-1β水平成正相关,与RSV患者严重程度密切相关,二者联合对RSV严重程度的预测效果较好。
Objective To investigate the expression and clinical significance of serum microRNA-19a-3p(miR-19a-3p)and suppressors of cytokine signaling 1(SOCS1)in children with respiratory syncytial virus(RSV)lower respiratory tract infection.Methods A total of 180 children with respiratory tract infection received in our hospital from January 2019 to January 2022 were selected as the study objects.The children with positive RSV RNA results(80 cases)were taken as the RSV infection group,and the children with negative RSV RNA results(100 cases)were taken as the control group.The RSV infection group was further divided into the severe group(Lowell score≥10 points)and the mild group(Lowell score<10 points)according to Lowell score at admission.The levels of miR-19a-3p and SOCS1 mRNA in blood were detected by real-time quantitative PCR.Pearson method was used to analyze the correlation between miR-19a-3p and SOCS1 expression in children’s serum.Results There were obvious differences between the two groups in clinical manifestations(fever,shortness of breath,wheezing,vomiting and diarrhea)and imaging chest films(scattered patchy shadows)(P<0.05).Compared with the control group,the levels of IL-6,TNF-αand IL-1βin serum of patients in RSV infection group were obviously higher(P<0.05).Compared with the control group,the levels of miR-19a-3p and SOCS1 in serum of patients in the RSV infection group were obviously higher(P<0.05).Compared with the mild group,the levels of serum miR-19a-3p and SOCS1 in the severe RSV group were obviously higher(P<0.05).Serum miR-19a-3p in RSV patients was positively corre-lated with SOCS1,IL-6,TNF-α,and IL-1βlevels(r=0.701,0.557,0.569,0.613,P<0.05).SOCS1 was positively correlated with the levels of IL-6,TNF-αand IL-1β(r=0.592,0.571,0.564,P<0.05).The ROC results showed that the levels of miR-19a-3p and SOCS1 in the serum of RSV patients,as well as the AUC predicted by the combination of the two for the severity of RSV infection,were 0.714,0.735,and 0.851,respectively.The combined prediction of AUC was significantly higher than the single prediction of AUC(Z_(combined with miR-19a-3p)=3.217,P<0.05)(Z=_(combined with SOCS1)=3.679,P<0.05).Conclusion The levels of miR-19a-3p and SOCS1 in blood of RSV patients are high,and they are positively correlated,also they are positively correlated with the levels of IL-6,TNF-αand IL-1βin RSV patients,and closely correlated with the severity of RSV patients.The combination of the two can better predict the severity of RSV.
