2型糖尿病住院患者营养状况分析及影响因素 被引量:1

Nutritional Status and Influencing Factors in Hospitalized Patients with Type 2 Diabetes Mellitus

摘要 目的使用控制营养状况(CONUT)评分法对2型糖尿病(T2DM)住院患者的营养状况进行评估并分析其影响因素。方法收集2021年1月至2022年12月在北京市昌平区中西医结合医院内分泌科住院的T2DM患者359例,根据CONUT评分法将患者分为营养正常组和营养不良组,分析两组患者基本情况和实验室检测指标的差异,采用logistic回归对T2DM患者营养不良的影响因素进行分析。结果359例T2DM住院患者中,营养正常组244例(68.0%);营养不良组115例(32.0%),其中轻度营养不良114例(99.1%),中度营养不良1例(0.9%)。营养不良组患者的年龄、中性粒细胞/淋巴细胞计数比值(NLR)与T2DM病程相较营养正常组均偏高(P<0.05);在糖化血红蛋白(HbA1c)、总蛋白(TP)、血红蛋白(Hb)、甘油三酯(TG)、尿酸与低密度脂蛋白(LDL)方面,营养不良组患者相较营养正常组均偏低(P<0.05)。多因素logistic回归显示:NLR水平为T2DM住院患者出现营养不良的危险因素,LDL、TP与TG则属于保护因素(P<0.05)。结论T2DM住院患者营养不良发生率高,且受多种因素影响。CONUT评分法可快速对患者营养状况进行筛查,对临床开展相应干预给予指导,由此降低患者的不良并发症发生率。 Objective To assess the nutritional status of hospitalized patients with type 2 diabetes mellitus(T2DM)and analyze its influencing factors using the controlled nutritional status(CONUT)score.Methods A total of 359 patients with T2DM who were hospitalized in the Department of Endocrinology of Beijing Changping District Hospital of Integrated Traditional Chinese and Western Medicine from January 2021 to December 2022 were collected.According to the CONUT score standard,patients were divided into normal nutrition and malnutrition groups.The differences in basic information and laboratory test indicators between the two groups were analyzed,and logistic regression was used to analyze the influencing factors of malnutrition in patients with T2DM.Results Among the 359 hospitalized patients with T2DM,244(68.0%)were in the normal nutrition group.There were 115 cases(32.0%)in the malnutrition group,including 114 cases(99.1%)with mild malnutrition and 1 case(0.9%)with moderate malnutrition.The age,neutrophil to lymphocyte ratio(NLR),duration of T2DM of the malnutrition group were higher than those in the normal nutrition group(P<0.05).The levels of glycosylated hemoglobin(HbA1c),total protein(TP),hemoglobin(Hb),triglycerides(TG),uric acid and low density lipoprotein(LDL)were lower than those in the normal nutrition group(P<0.05).Multivariate logistic regression showed that NLR level was a risk factor for malnutrition in hospitalized patients with T2DM,and LDL,TP,TG were its protective factors(P<0.05).Conclusion The incidence of malnutrition in hospitalized patients with T2DM is high and affected by a variety of factors.The CONUT score can quickly screen patients for nutritional status and guide clinical interventions,thereby reducing the occurrence of adverse complications in patients.
