物理因子干预联合关节动能控制训练在膝关节骨性关节炎中的应用效果

Application Effect of Physical Factor Intervention Combined with Joint Kinetic Energy Control Training in Knee Osteoarthritis

摘要 目的探究关节动能控制训练在膝关节骨性关节炎(KOA)患者中的效果及对姿势控制能力、关节症状及关节活动度的影响。方法选取河南省直第三人民医院2021年2月至2022年10月127例KOA患者,根据随机数字表法分组。对照组63例接受超短波、磁疗、超声波等物理因子干预,观察组64例在对照组基础上联合关节动能控制训练,对比两组患者干预前后关节症状[西安大略大学和麦克马斯特大学关节炎指数评分(WOMAC)]、姿势控制能力(前后方向标准差、左右方向标准差、前后平均运动速度、左右平均运动速度、运动长度、运动面积)、关节活动度[膝关节主被动关节活动度(ROM)]。结果干预后观察组疼痛、关节僵硬、关节功能评分、前后方向标准差、左右方向标准差、前后平均运动速度、左右平均运动速度、运动长度、运动面积低于对照组,主被动ROM均高于对照组,差异有统计学意义(P<0.05)。结论关节动能控制训练联合超短波、磁疗、超声波等物理因子对KOA患者干预,可有效改善膝关节症状,提高膝关节活动度和姿势控制能力。 Objective To explore the effect of joint kinetic energy control training on patients with knee osteoarthritis(KOA)and its influence on postural control,joint symptoms and range of motion.Methods A total of 127 patients with KOA in the Third People’s Hospital of Henan Province from February 2021 to October 2022 were selected and grouped according to random number table method.Sixty-three patients in the control group received physical factors such as ultra-short wave,magnetic therapy and ultrasound,and 64 patients in the observation group received joint kinetic energy control training on the basis of control group.The joint symptoms[Western Ontario and McMaster University osteoarthritis index(WOMAC)],postural control ability(standard deviation of front and rear direction,standard deviation of left and right direction,average speed of front and rear motion,average speed of left and right motion,length of motion,area of motion),range of motion[active and passive range of motion(ROM)]of the two groups before and after intervention were compared.Results After intervention,pain,joint stiffness,joint function score,standard deviation of anteroposteric direction,standard deviation of left and right direction,mean anteroposteric motion speed,mean anteroposteric motion speed,length of motion and area of motion in observation group were lower than those in control group,and active and passive ROM were higher than those in control group,with statistical significance(P<0.05).Conclusion Joint kinetic energy control training combined with ultrashort wave,magnetic therapy,ultrasonic and other physical factors can effectively improve knee symptoms,knee motion and postural control ability in patients with KOA.
