摘要 目的分析载脂蛋白B(apoB)与载脂蛋白A1(apoA1)比值(apoB/apoA1)对系统性红斑狼疮(SLE)活动性预测价值的研究。方法回顾性分析2019年5月~2022年8月间某院住院治疗的SLE患者120例,根据系统性红斑狼疮疾病活动度评分(SLEDAI),将患者分为活动期(n=55)、稳定期(n=65),分析apoB/apoA1对活动性SLE的影响和预测价值。结果单因素和多因素Logistic回归分析显示:apoB/apoA1、血沉(ESR)、抗双链DNA(抗-dsDNA)、补体3(C3)均为活动性SLE的影响因素,OR值分别为4.119、1.257、3.520、0.251。apoB、apoA1、apoB/apoA1预测活动性SLE的曲线下面积(AUC)分别为0.610、0.664、0.732,对重度活动性SLE的AUC分别为0.621、0.645、0.728,apoB/apoA1预测活动性SLE、重度活动性SLE的AUC均高于apoB、apoA1,差异有统计学意义(均P<0.05)。apoB、apoB/apoA1与SLEDAI均呈正相关,相关系数(r)值为0.285、0.454,apoA1与SLEDAI呈负相关,r值为-0.376。结论apoB/apoA1为活动性SLE独立影响因素,且对活动性、重度活动性SLE有较好预测价值。 Objective To analyze the predictive value of apolipoprotein B(apoB)to apolipoprotein A1(apoA1)ratio(apoB/apoA1)on systemic lupus erythematosus(SLE)activity.Methods 120 SLE patients hospitalized in our hospital from May 2019 to August 2022 were retrospectively analyzed.According to the SLEDAI,the patients were divided into active(n=55)and remission(n=65)phases,and the impact and predictive value of apoB/apoA1 on active SLE were analyzed.Results The univariate and multivariate logistic analysis showed that apoB/apoA1,erythrocyte sedimentation rate(ESR),anti-dsDNA(anti-dsDNA)and complement 3(C3)were the influencing factors of active SLE,with OR values of 4.119,1.257,3.520 and 0.251 respectively.The area under the curve AUCs of apoB,apoA1 and apoB/apoA1 predicting active SLE were 0.610,0.664 and 0.732 respectively,and the AUCs of severely active SLE were 0.621,0.645 and 0.728 respectively.The AUC of apoB/apoA1 in predicting active SLE and severe active SLE was higher than that of apoB and apoA1,and the differences were statistically significant(all P<0.05).ApoB,apoB/apoA1 were positively correlated with SLEDAI,and the correlation coefficient(r)values were 0.285 and 0.454.ApoA1 was negatively correlated with SLEDAI,and the r value was-0.376.Conclusion ApoB/apoA1 is an independent influencing factor for active SLE and has good predictive value for active and severely active SLE.
