摘要
目的探讨对社区中年高血压患者实施医学生联动健康管理模式的应用效果。方法病例采集时间2019年1月-2022年1月,对象为社区服务中心已建档的80例中年高血压患者,由医学生参与制定健康管理方案并对患者进行健康管理;利用问卷调查和访谈评估对比医学生参与前、后患者的血压控制效果、自我效能感、自我管理能力、生活质量、治疗依从性。结果管理后患者收缩压、舒张压水平均低于管理前(P<0.05);自我管理相关知识、健康相关行为、健康管理相关活动评分均高于管理前(P<0.05);管理后患者GSES的自我认识、诊疗环境、诊疗时控制感、自信心评分均显著高于管理前(P<0.05);患者在规范用药、健康饮食、运动锻炼、自我监测、情绪调节方面的依从性评分较管理前显著升高(P<0.05);管理后患者生活质量即SF-36评分较管理前显著升高(P<0.05)。结论开展医学生联动健康管理模式可提高社区中年高血压患者的血压控制效果,增强自我管理能力,同时全面提升其生活质量,提高患者预后水平。
Objective To explore the application effect of medical student linkage health management model for middle-aged hypertensive patients in community.Methods From January 2019 to January 2022,the cases were collected from 80 middle-aged hypertensive patients who had been filed in the community service center.Medical students participated in the formulation of health management plans and carried out health management for patients.Using questionnaire and interview to evaluate and compare the blood pressure control effect,self-efficacy,self-management ability,quality of life and treatment compliance of medical students before and after participation.Results After management,the systolic and diastolic blood pressure levels were lower than those before management(P<0.05).The scores of selfmanagement related knowledge,health-related behaviors and health-related activities after management were higher than those before management(P<0.05).The scores of self-awareness,sense of control and self-confidence in GSES after management were significantly higher than those before management(P<0.05);the compliance of patients in regulating medication,healthy diet,exercise,self-monitoring and emotion regulation were significantly higher than those in management(P<0.05);the quality of life(SF-36 score)after management was significantly higher than that before management(P<0.05).Conclusion The health management model of medical students can improve the blood pressure control effect of middle-aged hypertension patients in community,enhance their self-management ability and improve their quality of life in an all-round way,so as to improve the prognosis of patients.
