摘要
目的分析易误诊的特殊乳腺癌细胞病理形态学特征,以期提高乳腺癌细胞病理学诊断水平。方法回顾性分析2019年1月1日至2020年4月1日于本院行细针吸取细胞学(FNAC)及组织病理检查的351例乳腺肿块患者的临床资料,比较FNAC及组织病理检查结果,选取诊断不一致的4例患者,分析液基细胞学染色、病理检查及免疫组化结果。结果351例乳腺肿块患者中,4例误诊为良性病变,病理学检查诊断为恶性病变,分别为乳腺浸润性癌1级(非特殊型)、浸润性小叶癌、浸润性乳头状癌、乳腺浸润性癌2级(非特殊型)。结论4例患者的细胞穿刺均显示肿块细胞中等大小,异型性小,容易误诊;在乳腺低级别肿瘤中,肿瘤细胞排列方式、核浆比及细胞特征对诊断肿块性质具有重要意义。
Objective To analyze the cellular pathomorphological characteristics of special breast cancer easily misdiagnosed,in order to improve the level of cellular pathological diagnosis of breast cancer.Methods The clinical data of 351 patients with breast masses who underwent fine needle aspiration cytology(FNAC)and histopathological examination in our hospital from January 1,2019 to April 1,2020 were retrospectively analyzed.The results of FNAC and histopathological examination were compared,4 patients with inconsistent diagnosis were selected to analyze the results of liquid-based cytology staining,pathological examination and immunohistochemistry.Results Among the 351 patients with breast masses,4 cases were misdiagnosed as benign lesions,and the histopathological examination was diagnosed as malignant lesions,which were invasive breast cancer grade 1(non-special type),invasive lobular carcinoma,invasive papillary carcinoma and invasive breast cancer grade 2(non-special type).Conclusion The cell puncture of 4 patients showed that the tumor cells were of medium size,with small atypia and easy to be misdiagnosed;in low-grade breast tumors,tumor cell arrangement,nuclear-cytoplasmic ratio and cell characteristics are of great significance for the diagnosis of tumor properties.
