摘要
目的观察基于“筋滞骨错”理论推拿治疗膝骨关节炎的临床疗效。方法将102例膝骨关节炎患者按照随机数字表法分为2组,在均予西医常规治疗的基础上,对照组51例予常规推拿治疗,治疗组51例基于“筋滞骨错”理论推拿治疗。2组均治疗4周后统计疗效,比较2组治疗前后中医症状评分、西安大略和麦克马斯特大学(WOMAC)骨关节炎指数评分及患膝关节液基质金属蛋白酶3(MMP-3)水平、患膝周围肌群[包括半腱肌(ST)、股二头肌(BF)、股外侧肌(VL)、股直肌(RF)、股内侧肌(VMO)]肌力变化情况。结果治疗组总有效率92.16%(47/51),对照组总有效率74.51%(38/51),治疗组疗效优于对照组(P<0.05);与本组治疗前比较,2组治疗后中医症状评分、WOMAC骨关节炎指数评分及关节液MMP-3水平均降低(P<0.05),且治疗组治疗后中医症状评分、WOMAC骨关节炎指数评分及关节液MMP-3水平均低于对照组(P<0.05)。与本组治疗前比较,2组治疗后患膝周围肌群ST、BF、VL、RF、VMO肌力水平均升高(P<0.05),且治疗组治疗后患膝周围肌群ST、BF、VL、RF、VMO肌力水平均高于对照组(P<0.05)。结论基于“筋滞骨错”理论推拿治疗膝骨关节炎疗效确切,可有效改善患者中医症状,促进膝关节功能恢复,其作用机制可能与降低关节液内MMP-3水平,促进周围肌群肌力恢复有关。
Objective To explore the effect of massage in the treatment of knee osteoarthritis Guided by the theory of“JinzhiGucuo”.Methods A total of 102 patients with knee osteoarthritis in our hospital were randomly allocated to the control group(n=51)and the treatment group(n=51).The control group was given conventional massage,and the treatment group was given massage guided by the theory of"JinzhiGucuo".A treatment was performed for four weeks,aiming to compare clinical efficacy,traditional Chinese medicine(TCM)symptom score,Western Ontario and McMaster Universities Osteoarthritis Index(WOMAC)score,matrix metalloproteinase-3(MMP-3)level in knee joint fluid,as well as muscle strength of peripheral muscle groups including semitendinosus(ST),biceps femoris(BF),vastus lateralis(VL),rectus femoris(RF)and vastus medialis(VMO)between groups before and after treatment.Results The total effective rate of the treatment group was higher than that of the control group(92.16%vs 74.51%P<0.05).After treatment,the TCM symptom score,WOMAC score and MMP-3 level in the two groups were decreased(P<0.05),which were lower in the treatment group than in the control group(P<0.05).The muscle strength levels of ST,BF,VL,RF and VMO in the two groups were increased(P<0.05),which in the treatment group were more prevalent(P<0.05).Conclusion Guided by the theory of"JinzhiGucuo",Massage therapy has effective on knee osteoarthritis,which can effectively improve TCM symptoms and promote the recovery of knee joint function.and it promote muscle strength recovery of surrounding muscle groups by reducing the level of MMP-3 in synovial fluid.
