摘要
目的探讨腰椎间盘突出症中CT与MRI检查的诊断价值。方法选取2020年2月—2022年2月邹城市中医院收治的100例疑似腰椎间盘突出症患者,均接受CT、MRI检查及手术病理检查,统计比较CT与MRI检查对腰椎间盘突出症的诊断价值及对不同病变特征的诊断符合率。结果CT与MRI检查的灵敏性、特异性、准确性、阳性预测值、阴性预测值比较,差异无统计学意义(P>0.05)。CT检查的钙化诊断符合率为51.22%(42/82)高于MRI检查,椎间盘变形、脊髓变形的诊断符合率为58.43%(52/89)、4.35%(2/46)均低于MRI检查,差异有统计学意义(χ^(2)=26.889、9.818、71.704,P<0.05),但CT与MRI检查的椎间盘积气、神经根受压、硬膜囊受压诊断符合率比较,差异无统计学意义(χ^(2)=1.515、0.161、0.034,P>0.05)。结论腰椎间盘突出症中CT与MRI检查的诊断价值相当,但MRI较CT检查能够更好地显示椎管内结构。
Objective To investigate the diagnostic value of CT and MRI in lumbar disc herniation.Methods 100 suspected patients with lumbar disc herniation admitted to Zoucheng Traditional Chinese Medicine Hospital from February 2020 to February 2022 were selected,all of whom underwent CT,MRI examination,and surgical pathological examination.The diagnostic value of CT and MRI examination for lumbar disc herniation and the diagnostic accuracy of different pathological features were statistically compared.Results There was no statistically significant difference in sensitivity,specificity,accuracy,positive predictive value,and negative predictive value between CT and MRI examinations(P>0.05).The diagnostic accuracy of calcification in CT examination was 51.22%(42/82),which was higher than that in MRI examination,while the diagnostic accuracy of intervertebral disc deformation and spinal cord deformation was 58.43%(52/89)and 4.35%(2/46),which were lower than those in MRI examination,and the difference was statistically significant(χ^(2)=26.889,9.818,71.704,P<0.05).However,there was no statistically significant difference in the diagnostic accuracy between CT and MRI examination for intervertebral disc herniation,nerve root compression,and dural sac compression(χ^(2)=1.515,0.161,0.034,P>0.05).Conclusion The diagnostic value of CT and MRI in lumbar disc herniation is equivalent,but MRI can better display the structure of the spinal canal than.
